Electrophysiology procedures involving left atrium navigation are becoming more frequent, mostly due to the increase of atrial fibrillation ablation. Mapping catheters of different shapes and size as well as dedicated sheaths are mandatory tools for the accomplishment of procedural end point. Therefore, technical issues are expected, usually unrelated to significant risk. However, any accidental intra-atrial device loss of integrity implies a risk of cerebrovascular embolization. The lack of clear evidence on how to manage these events and the need for a quick solution complicate the scenario. We report an empirical solution in the case of debris floating in the left atrium.
Les interventions d’électrophysiologie avec navigation dans l’oreillette gauche sont de plus en plus courantes, surtout en raison du nombre croissant d’ablations pratiquées pour traiter une fibrillation auriculaire. Des cathéters de cartographie de tailles et de formes variables et des gaines spéciales sont indispensables pour mener l’intervention à bien. Il faut donc s’attendre à rencontrer des problèmes techniques, qui ne posent habituellement pas un risque significatif. Toute perte d’intégrité accidentelle d’un outil utilisé à l’intérieur de l’oreillette comporte néanmoins un risque d’embolie vasculaire cérébrale. L’absence de données précises sur la manière de prendre en charge ces événements et la nécessité de trouver rapidement une solution compliquent la situation. Notre article fait état d’une solution empirique employée pour éliminer un débris en suspension dans l’oreillette gauche.
