Sur la base de résultats intéressants, publiés dans certains ouvrages par plusieurs chercheurs des années 60 à nos jours, quant à l'utilisation de substances adhésives, nous avons voulu tester une colle spéciale au pouvoir adhésif élevé et constituée de monomères d'alpha-cyanacrylate étyle-2-alpha-cyanacrylate. Pour ce faire nous avons utilisé, comme animaux de laboratoire, 15 poulets adultes d'environ 4 mois et pesant 2 kg, que nous avons divisés en 5 groupes de 3 poulets chacun. Tous les animaux ont subi une intervention pour la préparation d'un tasseau osseux rectangulaire, placé au niveau du tiers moyen-proximal du tibia, pour son isolation complète, sa remise en place et sa stabilisation avec de l'adhésif + du collagène. A des dates étabies (7-14-21-30 et 60 jours), tous les animaux ont été tués, après avoir passé une radiographie, et la partie anatomique concernée a été étudiée du point de vue macroscopique et microscopique. A l'aide de cette expérimentation, nous avons voulu tester la capacité de l'adhésif cyanacrylate à maintenir la réduction de petits fragments osseux instables. Les résultats obtenus ont permis de confirmer une bonne stabilité du fragment, une bonne consolidation osseuse, ainsi qu'une absence de rejet. A la suite de ces excellents résultats, nous avons décidé d'utiliser cet adhésif en Chirurgie de la Main. Toutefois, nous pensons qu'il est indispensble de poursuivre les essais expérimentaux, afin d'avoir de meilleures garanties quant aux bons résultats obtenus.
Following certain interesting results reported in publications by researchers from the nineteen-sixties up to the present time, we decided to experiment a particular adhesive with high adhesive properties : this is composed of monomers of alpha-cyanoacrylate éthyl-2-alpha-cyanoacrylate. For this end, experiments were performed on 15 adult chickens, aged around 4 months, weighing 2 kg, divided into 5 groups of 3 chikens each. In each chicken, a rectangular bone fragment from the proximal-middle third of the tibia was prepared, completely separated, and then subsequently re-positioned and fixed with the adhesive plus collagen. At pre-established intervals (7, 14, 21, 30 and 60 days), each group of chickens was examined by x-rays and then killed, so that the particular anatomical part under examination could be studied both microscopically and macroscopically. The aim of these experiments was to evaluate the ability of cyanoacrylate adhesive to ensure setting of small bone-fragments that would otherwise be unstable. The results obtained showed the following : good stability of the bone fragment ; good bone consolidation ; absence of foreign body reactions. The positive nature of these results leads us to propose the use of this adhesive in hand surgery. Nevertheless, duty requires that further experiments be performed in order to acquire even greater guarantees for the positive results obtained.
En base a los interesantes resultados expresados en estudios publicados por algunos investigadores desde los años 60 hasta hoy sobre la utilización de sustancias adhesivas, hemos deseado experimentar una cola especial con alto poder adhesivo, constituida por monómeros de alpha-cianoacrilato Etil-2-alpha-cianoacrilato. A tal fin hemos utilizado como animales de laboratorio 15 pollos adultos, de unos 4 meses, de 5 kg de peso, divididos en 5 grupos de 3 pollos cada uno. Todos los animales fueron sometidos a una intervención de colocación de un fragmento óseo triangular a nivel del 1/3 medio-proximal de la tibia, y a su total aislamiento. A continuación hemos procedido a su posicionamiento y estabilización con cola + colágeno. A períodos de tiempo preestablecidos (7-14-21-30 y 60 días), todos los animales fueron sacrificados, previo examen radiográfico, y la pieza anatómica gue estudiada desde el punto de vista macroscoópico y microscópico. Con est trabajo experimental, se ha querido evaluar la bueno capacidad de la cola cianoacrilata de mantener la reducción de pequeños fragmentos óseos inestables. Lors resultados obtenidos han permitido verificar una bueno estabilidad del fragmento, una bueno consolidación ósea y una ausencia de reacciones de cuerpo extraõ. Dado que los resultados han sido interesantes, tenemos intención de usar dicho adhesivo en Ciruía de la Mano. No obstante, pensamos que es indispensable continuar los ensayos experimentales a fin de tener mejores garantiías sobre los buenos resultados obtenidos.
