La présence de lésions athéromateuses carotidiennes est fréquente chez les patients diabétiques. Par ailleurs, si le risque d’accident vasculaire cérébral est augmenté chez les patients avec un athérome carotidien, le risque apparaît plus élevé chez les patients présentant des lésions évolutives. Un suivi de la progression de l’athérome pourrait être un élément utile pour cibler les patients les plus à risque. Nous avons analysé l’évolution des lésions athéromateuses carotidiennes par échographie doppler chez une cohorte de patients diabétiques.Trois cent quarante-deux patients diabétiques ont été inclus entre 1993 et 2010. Les résultats du doppler ont été classé en normal (Absence de lésion athéromateuse), lésion < 50 % (ratio de vitesse maximale systolique carotide interne/carotide externe < 2) et > 50 % (ratio de vitesse > 2). La progression a été définie pour un patient donné comme le passage à un degré de sévérité supérieur, une régression comme le passage à un degré inférieur. L’âge moyen au premier doppler était de 59,2±10,8 années et l’ancienneté du diabète de 13,6±11,4 années. Au total, sur l’ensemble du suivi, 1 014 dopplers ont été réalisés soit 3 dopplers par patient. Le délai moyen entre deux examens était de 2,7±2,1 années. Le suivi moyen a été de 6,4±4,6 années.Initialement 31,3 % des patients avaient un doppler normal, 64,6 % une lésion < 50 % et 4,1 % une lésion > 50 %. Une progression a été observée pour 20,1 % des patients avec un délai moyen de 6,5±5,3 années soit une progression annualisée de 3,1 %. Une régression a été constatée pour 11 % des patients. Aucun patient sans lésion initiale n’a développé au cours du suivi de lésion > 50 %. Six patients ont bénéficié d’une chirurgie carotidienne.L’évolution des lésions carotidiennes reste un élément fréquent dans la population diabétique. Toutefois les indications de chirurgie carotidienne restent rares et l’impact du suivi sur la prise en charge thérapeutique reste à démontrer dans une étude prospective.Les auteurs déclarent ne pas avoir d’intérêt direct ou indirect (financier ou en nature) avec un organisme privé, industriel ou commercial en relation avec le sujet présenté.
