Nous rapportons un cas mortel de dengue hemorragique, survenu durant la premiere epidemie documentee de dengue hemorragique en Martinique, en novembre 1995. Le patient etait traite au long cours par Aspegic(R) (acetylsalicylate de lysine) 325 mg.j - 1 apres un accident ischemique cerebral transitoire. Quelques jours apres avoir presente un syndrome pseudogrippal, il etait hospitalise pour une alteration de l'etat general. Une thrombopenie associee a une hemoconcentration etaient alors retrouvees. Une hematemese a entraine un etat de choc, associe a une acidose lactique majeure. Le patient est decede dans les 24 premieres heures de son hospitalisation. La serologie de la dengue etait positive en IgM. Le patient est decede d'une dengue hemorragique de degre IV selon la classification de l'Organisation mondiale de la sante (OMS). S'il est classique de conseiller la prudence dans l'utilisation des salicyles chez les malades presentant un syndrome pseudogrippal en periode d'epidemie, la conduite a tenir lors de traitements anticoagulants ou antiagregants au long cours n'est pas etablie.
We report a case of fatal dengue hemorrhagic fever in the first dengue epidemic in Martinique (French West Indies) in November 1995. The patient was administered a long-term ASPEGIC 325 mg a day, treatment after a transient cerebral ischemia. The first signs were an influenza-like syndrome, but he was hospitalized due to worsening of his clinical status. Thrombopenia and major hemoconcentration were then discovered. Hematemesis led to shock syndrome with a major lactic acidosis. The patient died after a 24 h clinical course. Dengue blood IgM antibodies were positive. According to WHO classification, the subject died due to level IV Dengue Hemorrhagic fever. Salicylates should be used prudently for patients with a influenza-like syndrome during epidemic periods. Use of long-term therapies with anticoagulants or anti-platelet drugs remains unclear.
