Depuis 1992, la technique du ganglion sentinelle s'est largement développée pour la stadification pathologique. L'évolution de cette technique s'est faite vers une augmentation du nombre de ganglions prélevés par rapport au principe initial, sans consensus. Le but de notre étude est de démontrer que l'on peut maintenir un staging pathologique fiable en prélevant un nombre minimum de ganglion.Nous avons réalisé une étude prospective chez 50 patients atteints de mélanome de stade I. Pour chaque patient le prélèvement du ganglion sentinelle a été réalisé par un simple marquage radioactif. Nous prélevons le ganglion le plus radioactif et ceux dont la radioactivité est supérieure à 70% du premier. Les résultats ont été appréciés sur les caractéristiques des mélanomes, le taux de détection, le taux de positivité et le nombre de ganglions prélevés par patients. Nos résultats sont comparés à ceux de deux grandes études par un test du χ 2 .Dans toutes les procédures, nous avons identifié un ganglion sentinelle; 1,3 ganglion a été prélevé par patients avec un taux de positivité du ganglion sentinelle de 22%. La comparaison par analyse statistique avec les deux études montre une plus grande sélectivité de notre étude, avec des résultats, en termes de récidive et de positivité du ganglion, similaires.Notre technique réduisant le nombre de ganglions prélevés est fiable. Un nombre minimum de ganglions n'altère pas la sensibilité du staging tout en réduisant le coût de la procédure et en revenant à l'esprit de cette technique.
Since 1992, sentinel node (SLN) biopsy was generally applied to melanoma for carcinologic staging. Literature points out an increase of nodes removed for each procedure. It means to a high cost for this procedure and it wanders from the defining concept of SLN. The aim of our study was to evaluate whether, we can minimize number of SLN removed, without influencing the reliability of carcinologic staging.We conducted a prospective study about 50 patients with stage one melanoma. For each patient, the SLN was identified with hand-held gamma probe technique. We removed only the hottest and all nodes greater than 70% of the hottest. We analysed the characteristics of melanoma, the success rate of this procedure, how many nodes have been removed and how many have had micro-metastases. This result was compared to two main studies with χ 2 test.The success rate of this technique was 100%. We dissected 1,3 SLN for each patient, with 22% positive SLN. Statistical analyse pointed out a better selectivity of our study, rate of pathological positivity and recurrence was alike.Our technique decreasing number of removed SLN is reliable. A minimal number of nodes doesn't distort sensitivity of carcinologic staging, and reduce cost of the procedure.
