Les leucémies congénitales surviennent préférentiellement chez les nouveau-nés atteints de trisomie 21. Il s’agit de leucémies aiguës myéloïdes (LAM) de type M7, dont la rémission spontanée est fréquente, d’où leur nom de leucémie transitoire (LT). Quelques cas ont été décrits sans anomalie constitutionnelle du chromosome 21 mais avec présence de ces anomalies au sein des blastes.Nous rapportons l’observation d’une LT avec épanchement péricardique isolé, chez un nouveau-né de phénotype normal. Les blastes exprimaient une trisomie 21. L’enfant est en rémission complète avec actuellement 32 mois de recul.Dans les leucémies congénitales, la présence d’une trisomie 21 sur le clone blastique est un facteur de bon pronostic et justifie de surseoir à une chimiothérapie de première intention.
Neonates with Down's syndrome have an increased risk for congenital leukaemia, particularly acute megakaryoblastic leukaemia (FAB, M7) which most often resolves spontaneously and is called transient leukaemia. It can be observed in non-constitutional trisomy 21 infants then presenting trisomy 21 on blasts cells.We report a transient leukaemia with an isolated pericardial effusion in a phenotypically normal neonate. Trisomy 21 was found on blasts cells. Complete remission remains after 32 months.Congenital leukaemias, with trisomy 21 on blasts cells have a good prognosis that justifies observation before using chemotherapy.
