La nutrition entérale (NE) est difficile à réaliser chez le patient de réanimation, car le dysfonctionnement du tube digestif est très fréquent et d'origine multifactorielle. Chez certaines catégories de patients comme les brûlés, la NE est d'autant plus difficile qu'elle est introduite tardivement, audelà de 18 heures. De plus, la NE est fréquemment interrompue, ce qui cause un retard d'apports important. Les patients reçoivent en moyenne 75 % de la quantité prescrite. Le support nutritionnel doit faire partie des priorités de prise en charge. L'instauration d'un contrôle quotidien des quantités de nutriments délivrées aux patients est essentielle pour réduire le risque de dénutrition La prescription systématisée, avec des protocoles de nutrition définis, augmente son efficacité. L'utilisation combinée de nutritions entérale et intraveineuse peut être nécessaire. Enfin, la remise en question permanente des résultats est essentielle pour une démarche de contrôle de qualité.
Efficient feeding in critically ill patients by the enteral route is difficult: gut dysfunction is frequent and multifactorial. In some categories, such as burn patients, delaying the introduction of enteral nutrition (EN) beyond 18 h further compromises the success of the technique. One of the major problems with EN in the ICU is that nutrition is frequently interrupted by various procedures, resulting in insufficient energy delivery. As a mean, ICU patients receive 75% of prescribed quantities. The first key to success is to think of nutrition as a priority. The daily control of quantities of nutrients delivered to the patients is important to avoid underfeeding and malnutrition. Introducing feeding protocols, and pre-printed prescription forms including nutritional support among other intensive therapy considerations, is an efficient way to improve delivery of nutrients. Prescribing prokinetics is helpful. Combining EN with parenteral nutrition may be required to reach the energy target. Nutritional support should be reassessed daily and included in the quality control of the unit.
