Les déformations congénitales de la paroi thoracique antérieure entraînent rarement une symptomatologie clinique organique. La principale indication opératoire est le retentissement psychologique chez l’adulte jeune. La technique d’ostéochondroplastie modelante avec stabilisation par attelle-agrafe peut être appliquée aux trois types de déformations, le pectus excavatum, le pectus carinatum et le pectus arcuatum. Elle nécessite une bonne connaissance anatomique de la malformation thoracique car elle va entraîner une modification majeure du thorax après réalisation de multiples ostéotomies et chondrotomies. Celles-ci constituent un véritable volet sternochondrocostal qui est ensuite stabilisé par ostéosynthèse à l’aide d’attelles-agrafes à glissière de Borrelly glissées en arrière ou en avant du sternum après correction, selon le type de la déformation. Cette technique, nécessitant une prise en charge pluridisciplinaire (anesthésistes, chirurgiens thoraciques, chirurgiens plasticiens et kinésithérapeutes) permet d’obtenir de bons résultats morphologiques, en redonnant à la paroi antérieure du thorax une anatomie normale.
Congenital malformations of the anterior chest wall rarely induce organic clinical symptomatology. The principal indication for surgery is the psychological impact for the young adult. The modelling technique referred as osteochondroplasty with stabilization by sliding splint staplers can be selected in the three following types of deformations: pectus excavatum, pectus carinatum and pectus arcuatum. Such procedure requires good anatomical knowledge of the thoracic malformation because it induces major modifications of the chest resulting from various osteotomies and chondrotomies. These latter interventions are aimed at constructing a real sternochondrocostal shutter which is stabilized afterwards by osteosynthesis using Borrelly’s splint staplers; these staples are positioned, after correction, behind or in front of the breastbone depending on the type of malformation. This technique that requires multidisciplinary management (anaesthetists, thoracic surgeons, plastics surgeons and kinesitherapists) allows obtaining good morphological results, by restitution of normal anatomic properties to the anterior chest wall.
