L'amylose est une pathologie délicate à diagnostiquer, souvent de diagnostic difficile, et l'atteinte cardiaque assombrit son pronostic.Nous rapportons le cas d'un homme de 72 ans consultant pour insuffisance cardiaque globale avec épanchement pleural. L'échocardiographie découvre une cardiopathie restrictive et oriente le diagnostic étiologique vers une amylose. La première atteinte histologique est pleurale. Le retentissement cardiaque est confirmé. Le diagnostic est étayé par des localisations digestive et cutanée. Il s'agit d'une amylose AL primitive. Un traitement par melphalan–dexaméthasone a permis une stabilisation à six mois.Il s'agit d'une observation originale d'amylose révélée cliniquement par l'atteinte cardiaque et histologiquement par l'atteinte pleurale. Le rôle diagnostique de l'échocardiographie en médecine interne est ici souligné.
Amyloidosis is often difficult to diagnose and cardiac involvement worsens the prognosis.We report the case of a 72-year old man consulting for cardiac failure with pleural effusion. A restrictive cardiomyopathy was discovered by echocardiography, and amyloidosis was then suspected. First histological localization was pleural. Cardiac involvement was confirmed. The diagnosis was supported by digestive and cutaneous localizations. It was an AL amyloidosis. Treatment with melphalan and dexamethasone allowed stabilization during more than six months.This is an original case report, because of the first clinical signs (cardiac failure), the histological proof (pleural histology). Echocardiography is particularly helpful in internal medicine.
