Nutritional intake appears to be an important factor contributing to aging. In the present study we describe changes in physical health related to nutritional intake among elderly persons in a 10-y longitudinal study. Among 304 healthy elderly participants (median age 72 y on entry into the study in 1980), 97 (34.2%) are still in good health 10 y later in 1990, 74 (26.5%) have become frail or sick, 54 (19.1%) have died, and 57 (20.1%) have dropped out of the study. Women with lower or higher energy intakes (in 1980 and 1981) than the current Recommended Daily Allowance (RDA; between 25 and 30 kcal/kg) were more likely to become frail or sick or to die in 1990 than those with energy intake in the midrange (below RDA, odds ratio (OR) = 3.3, confidence interval (CI) = 1.2-8.6; above RDA, OR = 3.4, CI: 1.1-10.7). Moreover, women with protein intakes greater than the midrange of 0.8-1.2 g/kg of body weight (1.20-1.76 g/kg in 1980 and 1981) tended to have fewer health problems over the next 10 y than those with protein intakes <0.8 g/kg, suggesting that the mean protein requirement in elderly adults is greater than that established by the 1985 joint World Health Organization/FAO/UNU Expert Committee. Moreover, a decrease in energy intake was greater in elderly persons who died or dropped out of the study because of illness.Les apports alimentaires jouent un role important au cours du vieillissement. Dans cette etude, nous decrivons les modifications de la sante physique observes au cours d'une etude longitudinale sur 10 ans chez des sujets ages. Parmi les 304 participants en bonne sante inclus dans l'etude (age median: 72 ans au debut de l'etude en 1980), 97 (34,2%) etaient en bonne sante 10 ans apres en 1990; 74 (26,5%) etaient affaiblis ou malades; 54 (19,1%) etaient decedes tandis que 57 (20,1%) etaient sortis de l'etude. Les femmes qui presentaient (en 1980 et 1981) des apports energetiques inferieurs ou superieurs aux AJR habituelles (25 a 30 kcal/kg) etaient les plus susceptibles de s'affaiblir, de devenir malade ou de deceder en 1990 que celles ayant des apports energetiques correspondant aux AJR moyens (< AJR : OR = 3,3, CI : 1,2-8,6; > AJR : OR = 3,4, CI : 1,1-10,7). De plus, les femmes dont les apports proteiques etaient superieurs a la fourchette moyenne de 0,8 a 1,2 g/kg de poids (1,2 a 1,76 g/kg en 1980 et 1981) tendaient a presenter moins de problemes de sante au cours des dix annees suivantes que celles ayant des apports proteiques inferieurs a 0,8 g/kg. Cela suggere que les besoins proteiques moyens des sujets ages sont superieurs a ceux etablis en 1985 par le comite d'experts FAO/OMS. Enfin, la reduction de l'apport energetique etait plus marque chez les sujets ages qui sont decedes ou qui sont sortis de l'etude a la suite d'une maladie.La ingesta nutricional parece ser un factor importante que contribuye al envejecimiento. En este estudio se describen cambios en el estado de salud fisica relacionados con la ingesta nutricional entre personas anosas sanos (mediana de edad al entrar en el estudio en 1980 = 72 anos), 97 (34.2%) todavia mantienen un buen nivel de salud 10 anos despues en 1990, 74 (26.5%) han enfermado, 54 (19.1%) han muerto y 57 (20.1%) han salido del estudio. Las mujeres con ingestas caloricas en 1980 y 1981, por encima o por debajo de la actual RDA ( Recommended Dietary Allowance = 25-30 Kcal/Kg), fueron mas propensas a enfermar o habian fallecido en 1990, que aquellas con una ingesta calorica dentro de ese rango (por debajo de la RDA: OR=3.3, IC=1.2-8.6; por encima de la RDA: OR=3.4, IC=1.1-10.7). Ademas las mujeres con ingestas proteicas por encima del rango de 0.8 a 1.2 g/Kg de peso corporal (1.2 a 1.76 g/Kg en 1980 y 1981) tendieron a tener menos problemas con su salud en los 10 anos siguientes que esas con ingestas proteicas por debajo de 0.8 g/Kg, sugiriendo que los requerimientos proteicos en adultos anosos es mayor que la establecida en 1985 por el comite conjunto de expertos OMS/ FAO / UNU . Ademas, la reduccion en la ingesta energetica fue mayor en los viejos que murieron o abandonaron el estudio por enfermedad.
