L'ajustement occlusaI a pour but d'obtenir une occlusion stable et fonctionnelle. L'examen anatomique et fonctionnel de l'occlusion est établi au travers de l'évaluation de la position mandibulaire (physiologique ou d'adaptation), de l'intercuspidation maximale (PIM), des surplombs et recouvrements (overjet et overbite) de toutes les dents, de l'étude de la profondeur cuspidienne et des courbes occlusales. La situation, la direction et la profondeur des facettes d'usure permettent de différencier l'usure physiologique, due à la mastication, de l'usure parafonctionnelle due au bruxisme. L'examen est complété par la recherche des interférences occlusales et des parafonctions orofaciales (Iinguales et labiales) qui peuvent compliquer le cadre cIinique. La valeur du décalage existant entre la position mandibulaire physiologique et la position d'intercuspidation habituelle conditionne la première phase thérapeutique. Si le décalage est égal ou inférieur à 1 mm, les précontacts sont effacés mais la PIM du patient conservée. Si le décalage est supérieur à 1 mm, la correction nécessite une modification de la PIM du patient. Cette première phase est suivie par l'ajustement des trajets fonctionnels et si nécessaire par la correction des fonctions orofaciales.
The goal, for occlusal adjustment, is to ensure stable and functional occlusion. Anatomical and functional examination of the occlusion is performed by assessing the mandibular position (physiological position or adaptative position), maximal intercuspidation, all teeth overjets and overbites, cuspid depth and occlusal curves. The localization, direction, and depth of wear facets allows differentiating mastication-induced physiological erosion from bruxism-induced para-functional erosion. As complementary examination, a screening for occlusal interferences and orofacial (lingual and labial) parafunctions that may constitute a complication of the clinical presentation. The value of the shift between the physiological mandible position and the habitual intercuspidation position is the basis of the first therapeutic phase. In case of a shift ≤ 1 mm, precontacts are removed but the maximal intercuspidation is preserved. If the shift is > 1 mm, correction necessitates a modification of patient's maximal intercuspidation. This first phase is followed by the adjustment of functional courses and, if necessary, by the correction of orofacial functions.
