Évaluation de l’impact de la TEP/TDM à la 18 FCholine (FCh) dans la prise en charge du cancer prostatique à risques élevé et intermédiaire lors du staging initial et de la récidive biologique, chez des patients sélectionnés en fonction de l’intention de traitement.Étude prospective de 84 patients porteurs d’un adénocarcinome prostatique dont l’indication de la TEP FCh était posée en réunion multidisciplinaire selon les caractéristiques des patients, de l’imagerie standard et des possibilités thérapeutiques. Quatre-vingt-six TEP ont été réalisées après injection de 4MBq/kg de FCh dans le cadre du bilan d’extension initial (BEI) (43 %/37 TEP) et de la rechute biologique (57 %/49 TEP). L’âge, le taux de PSA et le score de Gleason moyens étaient, respectivement, de 63ans (48–76), 16ng/mL (2,42–55) et 8 (6–10) dans le BEI et de 71ans (59–82), 4,9ng/mL (0,12–32,8) et 7 (6–9) dans la récidive biologique.Dans le cadre du BEI, la lésion primitive a été identifiée chez tous les patients, avec dans 23 cas (62 %) une maladie localisée à la prostate, une extension ganglionnaire locorégionale dans 8 cas (22 %) et la présence de métastases dans 6 cas (16 %). La TEP a confirmé l’intention thérapeutique dans 49 % des cas et entraîné une modification de la stratégie de traitement dans 43 % des cas en modifiant les champs d’irradiation (25 %) et en évitant la chirurgie dans 25 % des cas. Dans le cadre de la récidive biologique, la TEP objectivait une récidive locale dans 14 cas (29 %), locorégionale dans 10 cas (20 %) et la présence de métastases dans 18 cas (37 %). Elle n’a pas permis d’identifier la récidive dans 7 cas (14 %). La TEP FCh a confirmé l’intention thérapeutique dans 24 % des cas et a entraîné une modification de traitement dans 61 % des cas.Une sélection rigoureuse des patients permet d’améliorer l’impact de la TEP/TDM à la FCh sur leur prise en charge. Dans notre étude, cette sélectivité permet de modifier le traitement chez 55 % des patients.
