Nous avons étudié l'observance d'une prophylaxie primaire chez des patients à un stade avancé de l'infection par le virus de l'immunodéficience humaine (VIH) polymédicamentés depuis longtemps. Quatre-vingt-dix patients ayant moins de 100 lymphocytes CD4/μl ont reçu de la rifabutine (300 mg/j) en prophylaxie primaire anti-Mycobacterium avium intracellulare. Des dosages plasmatiques aléatoires de rifabutine ont été réalisés par chromatographie liquide de haute performance. Cinquante-neuf dosages ont été effectués chez 41 patients (17 hétérosexuels, 16 toxicomanes, 7 homo-bisexuels). La résiduelle était supérieure à 20 μg/l dans 53 dosages (89,8 %) chez 35 patients (85,4 %); elle était comprise entre 8 et 20 μg/l dans 2 dosages (3,4 %) chez 2 patients (4,8 %) et était indécelable dans 4 prélèvements (6,8 %) chez 4 patients (9,7 %). Le jour du dosage le nombre moyen de spécialitś pris par patient est de 6 (extrêmes 3–13) regroupant : anti-rétroviraux 1,27 (0–2), prophylaxies 3,27 (2–6), autres traitements 1,53 (0–5). Le nombre moyen de gélules absorbé par jour et par patient est de 14,3 (4–30) dont : anti-rétroviraux 4,22 (0–10), prophylaxies 6,73 (3–21), autres 3,39 (0–12). En conclusion, chez des patients polymédicamentés on obtient une observance supérieure à 85 % dans une population composée majoritairement d'hétérosexuels et de toxicomanes. Il faut toutefois tempérer ces résultats favorables en rappelant que cette étude a été réalisée en partie avant le début des bithérapies et en totalité avant l'apparition des anti-protéases.
The authors lead a study on compliance to primary MAC prophylaxis in patients with advanced HIV infection. Ninety patients with CD4 cell counts below 100/μl, received rifabutin (300 mg/d) in a primary prophylaxis trial for MAC bacteremia. Blood samples were randomly obtained and rifabutin concentrations measured by high-performance liquid chromatography. Fifty-nine blood samples from 41 patients (17 heterosexuals, 16 intravenous drug users, and 7 bisexual men) were assayed. Trough rifabutin plasma levels were: above 20 μg/l in 53 samples (89.8 %) from 35 patients (85.4 %); between 8 and 20 μg/l in 2 samples (3.4 %) from 2 patients (4.8 %), and was absent in 4 samples (6.8 %) from 4 patients (9.7 %). On the day the blood sample was drawn, the mean number of medical drugs per patient per day was 6 (range 3–13): anti-retroviral therapy 1.27 (0–2), prophylactic regimen 3.27 (2–6), other treatments 1.53 (0–5). The mean numbers of pills per patient per day was 14.3 (4–30): anti-retroviral 4.22 (0–10), prophylactic regimen 6.73 (3–21), others 3.39 (0–12). We conclude that, in patients — mainly heterosexuals and intravenous drug users — with advanced HIV-infection, prescribed treatment regimen is usually well followed (over 85 %). However these results must be interpreted with caution since this study was carried out before bitherapy and protease inhibitors became standard therapy.
