But de l'étude. – L'imagerie par résonance magnétique du cœur a démontré son efficacité pour détecter les territoires nécrotiques et prédire les zones viables du myocarde après un infarctus. Le but de cette étude a été de comparer la masse nécrosée, évaluée par IRM, avec le pic de créatine kinase (CK).Matériel et méthode. – Une résonance magnétique a été réalisée chez 16 patients ayant présenté un premier infarctus du myocarde. Le protocole IRM a comporté une étude de la cinétique segmentaire du ventricule gauche puis une étude de rehaussement tardif réalisée 15 minutes après l'injection de gadolinium. L'analyse des images obtenues lors de cette phase tardive a permis une quantification de la masse nécrosée. Le pic de CK a été déterminé pour chaque patient.Résultats. – La régression comparant la masse nécrosée, estimée par IRM, et le pic de CK a montré une corrélation significative (r=0,56, p=0,0225). Accessoirement nous avons constaté une corrélation inverse et significative entre le pic de CK et la viabilité théorique telle que décrite pour un seuil inférieur ou égal à 25 % de rehaussement transmural (r=–0,51, p=0,0433).Conclusion. – Après un premier infarctus du myocarde, l'IRM avec injection de gadolinium permet la quantification précise de la masse de myocarde nécrosée en corrélation avec le pic de CK.
Aim. – To compare the assessment of infarct size using MRI and peak CK value.Population and methods. – 16 patients presenting with a first MI and seriall measurement of CK underwent gadolinium-enhanced MRI.Results. – A significant correlation was observed between both estimates of infarct size (r=0.56, p=0.0025). In addition, an inverse correlation was found between residual viability and peak CK (r=-0.51; p=0.043).Conclusion. – After a first episode of MI, infarct size estimate by MRI correlates with that determined by peak CK measurement.
