Left ventricular free wall rupture is a catastrophic mechanical complication of myocardial infarction. We present an 82-year-old woman with an anterolateral ST segment elevation myocardial infarction treated with thrombolysis. Because of unexplained hypotension, echocardiography was performed and contrast (Definity; Lantheus Medical Imaging) was used to improve visualization. Findings included a small- to moderate-sized circumferential pericardial effusion without frank tamponade, however, there was significant intramyocardial tracking of the contrast into the epicardial space, localized to the mid to apical portion of the anterior septum, consistent with rupture or disruption of the wall segment. The patient was promptly taken to the operating room where fresh blood and clots were evacuated from the pericardial space with immediate hemodynamic improvement. The patient underwent successful surgical repair.
La rupture de la paroi libre du ventricule gauche est une complication mécanique catastrophique de l'infarctus du myocarde. Ici, nous présentons le cas d’une femme âgée de 82 ans traitée par thrombolyse pour un infarctus du myocarde avec sus-décalage du segment ST en antérolatéral. En raison d’une hypotension inexpliquée, une échocardiographie a été réalisée, en utilisant un agent de contraste (Definity; Lantheus Medical Imaging) afin d’améliorer la visualisation. Les observations comprenaient un épanchement péricardique circonférentiel de taille petite à moyenne sans tamponnade franche, cependant il y avait un trajet intramyocardique significatif du contraste jusque dans l’espace épicardique, localisé du milieu jusqu’à la partie apicale du septum antérieur, compatible avec une rupture ou une désintégration de la paroi. La patiente a été rapidement conduite en salle d'opération où du sang frais et des caillots ont été retirés de l'espace péricardique, suivi d’une amélioration immédiate des paramètres hémodynamiques. La patiente a ensuite subi une réparation chirurgicale réussie.
