La résection de la première rangée des os du carpe est une solution thérapeutique à l’arthrose radio-carpienne post-traumatique. Il est acquis que cette résection aboutit à une diminution de la force de poigne (Jebson et al., Richou et al.). Le but de ce travail était d’évaluer les résultats fonctionnelles et radiologiques chez les patients exerçant une activité professionnelle manuelle afin d’en préciser les indications.Entre 1995 et 2009, 23 résections de la première rangée des os du carpe ont été réalisées chez 21 patients présentant tous une activité manuelle. Il y avait 21 patients dont 5 femmes et 16 hommes de 53,7ans de moyenne d’âge. Pour 8 patients, un traumatisme ancien était retrouvé. Tous les patients étaient travailleurs manuels dont 11 manuels lourds. Le côté dominant était intéressé 11 fois. La plainte principale était la douleur. Il s’agissait de 14 SLAC (2 stades III, 11 stades II, 1 stade 1) et 9 SNAC (6 stades IIIB et 3 stades 2B). La voie d’abord était dorsale sous anesthésie locorégionale dans tout les cas, suivie d’une courte hospitalisation de 3,7jours (3 à 5jours). Les patients ont été immobilisés par une attelle de poignet pendant 1 mois.Trois patients ont présenté une algodystrophie résolutive dans l’année qui a suivie la chirurgie. Au recul moyen de 75,6 mois (minimum 24, maximum 153), 4 patients ont été perdus de vue. Un patient présentait, à 5ans de la chirurgie, une arthrose radio-carpienne nécessitant finalement la réalisation d’une arthrodèse radio-carpienne à 10ans de la résection. Il s’agissait d’un patient manuel lourd présentant une SNAC 3B. Parmi les 17 autres patients, l’EVA moyenne était de 2,2 ; 5 patients présentaient des douleurs résiduelles. L’arc de flexion/extension était similaire par rapport aux amplitudes préopératoires et correspondait à 67,2 % du côté controlatéral. La force de poigne était réduite de 34 % par rapport aux données préopératoires et correspondait à 72,6 % du côté controlatéral. Seize patients se sont dits satisfaits de l’intervention (11 très satisfaits). Il y a eu 3 invalidités professionnelles avec reconversion et 1 reclassement professionnel. Pour ces 4 patients, tous travailleurs manuels lourds, le motif retrouvé était le manque de force. Le score de QuickDASH moyen était de 26/100. Le PWRE retrouvé une EVA douleur à 2,8/10 et l’EVA fonction à 1,6/10. Lors du bilan radiologique à la révision, 3 patients présentaient un pincement radio-capitate par rapport aux radiographies post-opératoires. Il y a eu une diminution moyenne de la distance radio-capitate de face et de profil de 0,3mm. La hauteur du carpe a diminué en moyenne de 1mm sans translation notable du carpe.La résection de la première rangée des os carpe trouve son indication principale pour les SLAC/SNAC sans atteinte arthrosique du médio-carpe. Elle permet la conservation des mobilités préopératoires du poignet et une diminution des douleurs. Toutefois, elle s’accompagne d’une diminution notable de la force de poigne qui est responsable, dans notre étude, de 23 % de reclassement et reconversion professionnelle (1 patient sur 3 pour les travailleurs manuels lourds). Chez des patients ne présentant pas d’atteinte arthrosique du médio-carpe, si leur travail nécessite une activité de force importante alors l’arthrodèse médio-carpienne semble être une alternative de choix à la résection de la première rangée des os du carpe.
