En 2006, 6 509 nouveaux malades résidant dans 16 régions (Auvergne, Basse-Normandie, Bourgogne, Bretagne, Centre, Champagne-Ardenne, Corse, Haute-Normandie, Ile de France, Languedoc-Roussillon, Limousin, Lorraine, Midi-Pyrénées, Nord-Pas-de-Calais, Provence-Alpes, Côte d’Azur et Rhône-Alpes) couvrant 79 % de la population française (soit 48 M d’habitants) ont débuté un premier traitement de suppléance pour insuffisance rénale chronique avec un âge médian de 71 ans ; 3 % de ces malades ont bénéficié d’une greffe préemptive. L’incidence brute globale de l’insuffisance rénale terminale dans ces 16 régions était de 137 par million d’habitants. Il existait des différences significatives entre régions qui persistaient après standardisation sur le sexe et l’âge (de 101 à 179 pmh). À l’initiation du traitement, plus d’un malade sur deux avait au moins une comorbidité cardiovasculaire et 37 % un diabète (diabète type 2 dans 88 % des cas).Au 31/12/2006, 25 774 malades résidant dans les 16 régions considérées étaient en dialyse (âge médian 69,5 ans). Au 31/12/2006, on estimait à 21 328 le nombre de malades résidant dans ces 16 régions et porteurs d’un greffon rénal fonctionnel (âge médian 53 ans). La prévalence brute a été estimée pour les 15 régions ayant un enregistrement exhaustif à 536 par million d’habitants pour la dialyse et à 409 pour les malades porteurs d’un greffon rénal, soit une prévalence brute globale de 945 par million d’habitants, avec des différences entre régions persistant après standardisation (de 765 à 1 061 pmh).Dans la cohorte des 18 264 malades ayant débuté un traitement de suppléance entre 2002 et 2006, 82 % étaient en vie à 1 an, 72 % à 2 ans et 63 % à 3 ans. La probabilité de survie diminuait avec l’âge mais restait au-dessus de 50 % à 2 ans chez les malades de plus de 75 ans à l’initiation du traitement de suppléance.En 2006, parmi les 6 321 nouveaux malades traités par dialyse dans ces 16 régions, 6 % avaient un indice de masse corporelle inférieur à 18,5 kg/m 2 et 17 % un IMC supérieur à 30. À l’initiation du traitement de suppléance, 62 % avaient un taux d’hémoglobine inférieur à 11 g/l et 9 % une albuminémie inférieure à 25 g/l. La première hémodialyse a été effectuée en urgence pour 30 % des malades et dans 48 % des cas sur un cathéter.Au 31/12/2006, parmi les 25 558 malades traités dans les structures de dialyse de ces 16 régions, 8 % étaient en dialyse péritonéale (dont 38 % en dialyse péritonéale automatisée). En hémodialyse, 95 % des malades avaient eu 3 séances par semaine, d’une durée médiane de 4 heures.En 2006, 2 144 malades ont été greffés. Au 31/12/2006, 4 838 malades étaient en attente de greffe de rein dans les centres de transplantation des 16 régions.
In 2006, 6.509 patients with end-stage renal disease living in 16 regions covering 48 M inhabitants (79% of the French population), started renal replacement therapy (dialysis or preemptive graft): median age was 71 years; 3% had a preemptive graft. The overall crude annual incidence rate of renal replacement therapy for end-stage renal disease was 137 per million population (pmp) in 16 regions that met exhaustivity, with significant differences in sex and age-adjusted incidence across regions (107 to 179 pmh). At initiation, more than one patient out of two had at least one cardiovascular disease and 37% diabetes (88% Type 2 noninsulin-dependent diabetes).On December 31, 2006, 25.774 patients living in these 16 regions were on dialysis: median age was 69.5 years. On December 31, 2006, 19.491 patients were living with a functioning graft: median age was 53 years. The overall crude prevalence rate of dialysis was 536 pmp in 15 regions. The overall crude prevalence rate of renal graft was 409 pmp in 15 regions. The first haemodialysis was started in emergency in 30 % of the patients and with a catheter in 48%.On December 31, 2006, 8% treated in the dialysis units of the 16 regions received peritoneal dialysis, of which 38% were treated with automated peritoneal dialysis. 95% of the patients on haemodialysis had 3 sessions per week, with a median duration of 4 hours.In 2006, 2.144 patients received a renal graft. On December 31, 2006, 4.838 patients were on the waiting list for a renal graft in the transplantation centres of the 16 regions.
