La RCMI des cancers de prostate implique des contrôles quotidiens du positionnement de la prostate, possibles avec l’imagerie embarquée (scanographie de haute énergie). Ce repositionnement quotidien permet une augmentation de la précision du recalage. L’objectif était d’évaluer la dose délivrée aux organes à risque par les scanographies de haute énergie quotidiennes.Quarante-trois patients pris en charge par une tomothérapie de 80Gy en 40 fractions ont été inclus dans deux groupes : une scanographie de haute énergie quotidienne (40 scanographies de haute énergie) (groupe 1), et plus d’une scanographie de haute énergie par jour, avec une seconde scanographie de haute énergie après correction de la présence de selles et/ou de gaz (plus de 40 scanographies de haute énergie) (groupe 2). La dose apportée par chaque scanographie de haute énergie a été mesurée selon les recommandations de l’American Association of Physicists in Medicine (AAPM) Task Group 148. À partir de cette mesure, la dose maximale (Dmax) théorique et réelle reçues par le rectum et la vessie ont été recalculées, ainsi que la NTID (Normal Tissue Integral Dose) pour les tissus sains en prenant le cas théorique d’un patient recevant 0, 40, et 80 scanographies de haute énergie. Les toxicités ont été recueillies selon la Common Terminology Criteria for Adverse Events, version 4.0 (CTCAE v 4.01).La dose ajoutée par une scanographie de haute énergie est de 1 cGy. Dans le groupe 2, le nombre total de scanographies de haute énergie variait de 41 à 54. La Dmax délivréeé au rectum a été augmentée de 0,50Gy dans le groupe 1 et de 0,54Gy dans le groupe 2 (valeur moyenne). La NTCP (Normal Tissue Complication Probability) du rectum augmentait aussi en fonction du nombre de scanographie de haute énergie : les NTCP 5 %/5ans (risque de 5 % à 5ans) étaient respectivement de 9,09 %, 9,65 % 10,3 % pour 0, 40 et 80 scanographies de haute énergie. La NTID était de 111,5J, 112,1J et 112,6J, respectivement pour 0, 40 et 80 scanographies de haute énergie. Les toxicités aiguë et tardive étaient similaires entre les deux groupes.Il est important d’évaluer les doses délivrées par l’imagerie embarquée afin d’optimiser les procédures de radiothérapie guidée par l’image pour réduire la dose aux organes à risque.
