Cryptogenic organizing pneumonia (COP) is a rare entity in childhood, particularly in previously healthy children. Moreover, the spontaneous remission of the disease is exceptional. We report on the case of a previously healthy 10-year-old girl who consulted her doctor for a 2-month history of mild pyrexia, fatigue, weight loss, and exertional dyspnea with no response to amoxicillin. A chest CT revealed bilateral, asymmetrical peribronchovascular consolidation areas predominating in the lower lobes and imaging features of COP. The bronchoalveolar lavage and the biological findings were negative. On follow-up, the girl showed clinical and radiological remission of the disease with no treatment. Our report describes an extremely rare case of spontaneously resolving COP in a child, the diagnosis being made based on the imaging pattern.
La pneumonie organisée cryptogénique est rare chez l’enfant. Sa résolution spontanée est encore plus rare d’après la littérature. Nous rapportons le cas d’une fille de 10ans, sans antécédents particuliers, qui avait consulté son médecin pour de la fièvre avec fatigue, perte de poids et dyspnée à l’effort. Elle a reçu un traitement par amoxicillin sans amélioration. Le scanner thoracique a montré des condensations alvéolaires bilatérales péri-bronchovasculaires en faveur d’une pneumonie organisée. Le diagnostic a été confirmé par le tableau clinique et la négativité des résultats du bilan biologique et du lavage bronchoalvéolaire. Un diagnostic consolidé par les images du scanner de contrôle de l’enfant qui ont montré une régression complète en l’absence de tout traitement. Il s’agissait d’un cas rare de pneumonie organisée cryptogénique avec résolution spontanée.
