L'anevrisme des arteres de moyen calibre avec gangrene des extremites peut etre une complication grave de la maladie de Kawasaki.Observation. - Nous rapportons une nouvelle observation d'un garcon age de neuf mois. Le diagnostic etait fait un mois apres l'episode initial lors de l'apparition d'une ischemie de la main gauche. L'evolution etait marquee par l'apparition d'une gangrene et l'amputation des quatre derniers doigts malgre un traitement anticoagulant et fibrinolytique bien conduit.Conclusion. - Le pronostic de la maladie de Kawasaki depend de la rapidite de la prise en charge. Les nourrissons de moins de sept mois sont les plus susceptibles de developper des anevrismes coronaires geants et des arteres de moyen calibre. Ceux-ci justifient une surveillance rapprochee a la phase aigue et a moyen terme. En cas d'anevrisme coronaire geant, l'echographie doppler des axes vasculaires des membres doit etre d'indication facile et repetee. En cas d'apparition d'anevrisme des arteres de moyen calibre, un traitement anticoagulant doit etre instaure. Lorsque l'ischemie des extremites apparait, l'evolution se fait vers la necrose et l'amputation. Le traitement anticoagulant, vasodilatateur et fibrinolytique est le plus souvent decevant.
An aneurysm of the medium arteries is one of the major complications of Kawasaki syndrome. Eleven cases of giant coronary aneurysm have been reported. Case report. - We report a new case in a nine-month-old child. Ischemia of the left hand was the first symptom. Diagnosis was made 30<space>days later. Anticoagulant and fibrinolytic treatments did not prevent necrosis of four fingers. Conclusion. - The prognosis of Kawasaki disease depends on early diagnosis and management. Infants younger than seven months of age are more susceptible to developing coronary and mid-artery aneurysms, which therefore justifies a close observation during the acute phase and later on. In the case of a coronary artery aneurysm, Doppler echography of the upper and lower limbs is mandatorily recommended. Anticoagulant therapy should be started once the diagnosis of mid-artery aneurysms is made. Ischemia of the extremities can lead to necrosis and amputation. Until now, anticoagulants, vasodilators and fibrinolytic agents were not proven to be effective.
