Une anemie se definit au plan physiologique et pathogenique comme une diminution de la masse totale des hematies dans le sang circulant. Au plan clinique, une anemie se caracterise principalement par une paleur des muqueuses. D'autres signes cliniques peuvent etre observes et sont lies a l'hypoxie tissulaire ou au processus pathogenique ayant induit l'anemie. Pour un traitement efficace, un diagnostic etiologique doit etre etabli grace a une demarche clinique rigoureuse. Actuellement, ce diagnostic est facilite par l'augmentation du nombre d'automates d'hematologie disponibles pour les veterinaires et permettant de mesurer l'hemoglobinemie, le nombre d'hematies, le volume globulaire moyen et de calculer l'hematocrite, la concentration corpusculaire moyenne en hemoglobine et l'indice de distribution des hematies. Cependant, la premiere etape apres confirmation de l'anemie consiste a verifier son caractere regeneratif ou non regeneratif par la mesure du taux de reticulocytes et une observation attentive du frottis sanguin, riche en informations. Face a une anemie regenerative, il est necessaire de rechercher une hyperhemolyse ou une fuite sanguine. Face a une anemie non regenerative, il convient en revanche de classer les anemies normocytaires normochromes, microcytaires hypochromes ou macrocytaires hypo/normochromes afin d'aboutir au diagnostic etiologique.
Anaemia is an absolute decrease in red blood cells count. The first clinical sign of anaemia is the paleness of the mucosae. Other symptoms may be induced by hypoxia or by the cause of anaemia itself. A thorough clinical analysis resulting in a specific diagnosis is necessary for a complete, successful treatment. Fortunately, by using a systemic approach of the clinical presentation, the history, physical examination, and laboratory testing, making definitive diagnosis is easy most of the time. The increasing availability of semi-automated or automated haematology instrumentation allows more efficient testing than in the past. Automated cell counters determine red blood cells count, the mean corpuscular volume of the erythrocyte population and haemoglobin concentration and eventually calculate mean corpuscular volume, mean corpuscular haemoglobin concentration and the index of red cell distribution. However, the first step is to classify the anaemia as regenerative or non regenerative with blood smear evaluation and reticulocyte count. In regenerative anaemia, blood loss or haemolysis should be sought. In non regenerative anaemia, the aetiological diagnosis is based on the identification of normochromic normocytic anaemia, hypochromic microcytic anaemia, or hypo/normochromic macrocytic anaemia.
