La prothèse totale de cheville (PTC) est, avec l’arthrodèse de cheville, un traitement chirurgical radical des arthropathies invalidantes de l’articulation tibio-talienne. Ces deux techniques sont complémentaires, chacune ayant ses indications préférentielles respectives et ses points faibles. L’enfance du concept de PTC a été marquée par la succession de différents modèles jusqu’à arriver, en 1990, aux bases des implants actuels dits de « troisième génération ». L’adolescence a consisté à définir les modalités d’implantation tant du point de vue technique que dans la stratégie de l’indication. Les incertitudes à court et moyen termes paraissent réglées. La morbidité périopératoire est faible. L’amélioration de la douleur est la règle ainsi que la qualité de vie, même si la reprise des activités sportives n’est pas garantie. L’analyse de la marche montre un déroulement symétrique du pas. Cependant, la maturité n’est pas encore totalement acquise, le taux de survie se situant vers 80 % à 10ans. L’évolution à long terme peut être ternie, soit par des atteintes pouvant être traitées en conservant la prothèse (douleurs malléolaires, une rupture du patin polyéthylène, dégradation des articulations voisines), soit par des problèmes faisant envisager une fusion osseuse ou un changement d’implant (infection, anomalies de trame osseuse, mobilisations d’implants). Les voies de recherches visent à améliorer les implants actuels et à développer des implants de reprise.
The Total Ankle Arthroplasty (TAA) is, with the arthrodesis of ankle, a radical surgical treatment of the invalidating tibio-talar joint arthropathy. These two techniques are complementary, each having the respective preferential indications and its weak points. The childhood of the concept of TAA was marked by the succession of various models until arrive, in 1990, the bases of current “third generation” implants. The adolescence consisted in defining the modalities methods of setting-up so much technical point of view as in the strategy of the indication. The short and medium-term uncertainties seem to be adjusted. The perioperative morbidity is weak. The improvement of the pain is the rule as well as the quality of life, even if the resumption of the sports activities is not guaranteed. The analysis of the walking shows a symmetric progress of the step. However, the maturity is not still totally acquired, the rate of survival being situated towards 80% in 10 years. The long-term outcome may show problem, which can be treated with preservation of the TAA (malleolar pain, polyethylene breakage, degradation of the nearby joints). Some complications may lead to envisage an arthrodesis or make an implant replacement (infection, anomalies of osseous weft, mobilizations of implants). The ways of researches aim at improving the current implants and at developing implants of resumption.
