Background. - Steroids (hydrocortisone or ACTH) still remain the usual treatment for infantile spasms (IS). However, since 1990, some authors have reported the efficacy of vigabatrin (VGB), especially in cases related to tuberous sclerosis.Population and methods. - Seventy children with infantile spasms were treated by VGB first line monotherapy. Modalities of treatment and monitoring were the same for all children. VGB was given at the daily dose of 100 mg/kg during 1 week. If spasms persisted, the daily VGB dose was increased to 150 mg/kg during a 2nd week. In case of persistence of IS, on the 15th day, hydrocortisone was then added to VGB. Of the 70 infants, 39 were symptomatic and 31 cryptogenic.Results. - On VGB, 37 children (54%) stopped having IS within a mean 3.5 days. Response to VGB was different according to etiology. Among cryptogenic cases, 22 infants (71%) definitively stopped having spasms and only one relapsed. Among symptomatic cases, only 15 infants (38%) stopped having IS, and half (815) relapsed. VGB mean daily dose at cessation of spasms was 114 mg/kg. Side effects were transient drowsiness (27%) and agitation (12%). Mean follow-up was 10 months (1-24 months). Seventy-five percent of the infants presenting with a focus of spike after the 1st month of treatment relapsed.Conclusion. - Infants with cryptogenic spasms have a good response to VGB monotherapy. When mental retardation is noticed before IS, and MRI is normal, there is no efficacy. In these cases, the best treatment seems to be prolonged corticotherapy associated with VGB.Les steroides (ACTH ou hydrocortisone), associes ou non au valproate de sodium, sont encore actuellement le traitement de reference des spasmes infantiles. A partir de 1990, certains auteurs ont rapporte l'efficacite du vigabatrin dans les spasmes infantiles et plus particulierement dans la sclerose tubereuse de Bourneville.Patients et methode. - Soixante-dix nourrissons souffrant de spasmes infantiles ont ete traites par vigabatrin de premiere intention et en monotherapie. Le vigabatrin etait administre a la dose de 100 mg/kg/j pendant 8 jours, puis augmente a la dose de 150 mg/kg/j pendant 8 jours en cas d'echec. L'hydrocortisone etait ajoutee, au 15 e jour de traitement par vigabatrin, en cas de persistance des spasmes. Les 70 enfants etaient repartis en deux groupes : 39 cas symptomatiques et 31 cas cryptogeniques.Resultats. - Trente-sept nourrissons ont cesse de faire des spasmes sous vigabatrin dans un delai moyen de 3,5 jours. La reponse au vigabatrin etait differente en fonction de la cause. Dans le groupe cryptogenique, 22 nourrissons ont cesse de faire des spasmes durablement et un nourrisson a rechute. En revanche, dans le groupe symptomatique, seulement 15 nourrissons ont cesse de faire des spasmes sous vigabatrin et huit nourrissons ont rechute. La dose moyenne de vigabatrin pour obtenir l'arret des spasmes etait de 114 mg/kg/j. Les effets secondaires etaient une somnolence (19 cas) et une agitation (8 cas) transitoires. Le recul moyen etait de 10 mois (1-24 mois). Les trois quarts des nourrissons qui avaient conserve un foyer de pointes a l'electroencephalogramme au-dela du 1 e r mois de traitement par vigabatrin en monotherapie ont rechute.Conclusion. - Contrairement aux nourrissons ayant des spasmes infantiles cryptogeniques, qui repondent bien au vigabatrin en monotherapie, les nourrissons ayant un retard mental avant l'apparition des spasmes mais une imagerie cerebrale normale ne repondent qu'exceptionnellement, mais semblent beneficier d'un traitement prolonge par corticoides associes au vigabatrin.
