Aim; Evaluation of the usefulness and safety of PPBD. Objectives:26cases of pancreatic stone treated by EPDBD in our hospital in these 7 years. Methods;41 cases of pancreatic stone were treated in our hospital;7 cases by ESWL alone,4 by endoscopy alone,30 by ESWL and endoscopy. In this third group,24 cases out of 30 were treated by EPDBD, a new therapeutic procedure. Its indication was for such cases as pancreatic stone which could not be removed by ESWL and EPST. The balloon catheter, which is usually used for bile duct dilation, was inserted deep into the pancreatic duct, and inflated in the orifice and narrow segments of the duct at 5-10 atom of pressure for 3 minutes. Results; After this treatment, narrow portions of the duct were dilated and then basket catheter could pass through narrow segments and we could remove the fragments easily. In some cases, fragments came out spontaneusly just after this procedure. All cases were treated by EPDBD successfully. After this treatment, overall stone free rate was 96% (25/41), pain free rate was 96%(25/26).The key point of this new procedure was how to insert the guide-wire deeply into the pancreatic duct. Complications were slight epigastralgia, slight bleeding from the orifice during the treatment and moderate -hyper- amylasemia for a few days after the treatment. But patients recovered quickly and no major problems occurred. After removal of stones, pancreatic functions improved in several cases (endocrine45%, and exocrine63%). Conclusions; EPDBD is a very useful and safe therapeutic method for the pancreatic stone when the stones remaine after ESWL+EPST. Moreover dilation of the narrow segment of Chronic pancreatitis will prevent the disease from getting worse and occurrence of stones in the future.
