Les malformations lymphatiques demeurent un challenge pour le chirurgien. Parmi tous les traitements entrepris, la chirurgie et la radiologie interventionnelle se révèlent les seules efficaces. La grande diversité clinique de ces lésions rend difficile l'établissement d'un schéma thérapeutique standard. Bien souvent, les complications révélatrices comme l'infection, le saignement, l'obstruction respiratoire ou l'impotence fonctionnelle imposent un traitement rapide. La collaboration pluridisciplinaire est impérative. À travers notre expérience et une revue de la littérature, nous décrirons les principes du traitement chirurgical des malformations lymphatiques ainsi que les techniques spécifiques à chaque localisation anatomique particulière.
Lymphatic malformations remain a therapeutic challenge. Many treatments by the past led to poor success. The wide variety of clinical presentations makes it difficult to outline specific management programs. Often, diagnosis or complication circumstances as infection, bleeding, airway obstruction, or handicap force acting. Thus, a specific follow-up joined to a multidisciplinary analysis are the key of well-planned surgery. Through our experience and a review of literature, we will describe principles of lymphatic malformations surgery and specific technics for specific locations.
