La rhinoconjonctivite allergique est un problème important de santé publique en Europe. Sa prise en charge repose sur l’éviction des allergènes, une thérapie symptomatique et l’immunothérapie spécifique. Les caractéristiques de Grazax ® , lyophilisat sublingual, permettent d’améliorer l’observance et la simplicité d’emploi. Cette étude a été réalisée pour évaluer la sécurité d’emploi de Grazax ® dans les conditions préconisées d’utilisation en situation de médecine ambulatoire.Étude française de phase IV, multicentrique, randomisée, ouverte dont le critère principal est l’évaluation de la tolérance immédiate et retardée.Cent trente centres ont inclus 628 patients, issus d’un registre exhaustif, traités pendant 5,5±1,9mois ; 11 % des patients sont sortis d’étude prématurément pour effet indésirable. Lors de la première prise, 14 patients (2,2 %) ont présenté une réaction durant plus de 30minutes ou nécessitant la prise d’un traitement symptomatique. Les effets à long terme les plus fréquents, essentiellement des symptômes locaux oraux et pharyngés, ont touché 33,3 % des patients ; 4,6 % ont signalé des événements gastro-intestinaux. Les patients asthmatiques ou polysensibilisés avaient un profil semblable de tolérance. Trois patients ont présenté quatre événements indésirables graves, tous résolus en fin d’essai. L’observance était bonne chez 71,8 % des patients. Plus de 85 % des patients et praticiens étaient satisfaits ou très satisfaits.La tolérance de Grazax ® s’est révélée conforme à l’autorisation de mise sur le marché, notamment chez les patients asthmatiques ou polysensibilisés. L’incidence des événements à la première prise confirme l’importance d’une surveillance initiale par un médecin expérimenté dans le traitement des allergies.
Allergic rhinoconjunctivitis is a major public health issue in Europe. Its management is based on avoidance of causative allergens, symptomatic therapy and specific immunotherapy (SIT). Grazax ® oral lyophilizate has been developed to offer a well documented and easiness new SIT formulation. This study was performed to document tolerability of Grazax ® in real life settings.French national, randomized, phase IV, open-label, multicentre study. The main criterion was the evaluation of tolerability (immediate and long term).One hundred and thirty centers included 628 patients coming from an exhaustive registry. Patients were treated for 5.5±1.9months. Drop out rate for adverse events (AE) was 11.1%. Immediate tolerability following the first intake requiring a symptomatic treatment or lasting more than 30minutes affected 14 (2.2%) patients. Long term safety for which the most frequent AE were local symptoms, oral and pharyngeal affected 33.3% of the patients; gastrointestinal manifestations affected 4.6% patients. Asthmatic or polysensitized patients had a similar safety profile. Three patients presented four serious adverse events, all resolved at the end of the trial. Compliance was high in more than 71.8% of patients, patients and investigators were satisfied or very satisfied in more than 85% of the cases.The tolerability profile was similar to that described in the product labelling, whether the patient was polysensitized or suffering of a mild to moderate controlled asthma or not. The incidence of mild AE at first intake confirms the importance of this intake under initial supervision of a physician experienced in allergy management.
