La chimiothérapie est le traitement de référence des tumeurs trophoblastiques gestationnelles. Leur chimiorésistance peut justifier le recours à l’hystérectomie malgré son impact sur la fertilité. Une nullipare de 31ans a présenté une môle invasive récidivante malgré deux lignes de chimiothérapie. Elle a subi une résection utérine partielle d’un choriocarcinome intramyométrial, suivie d’une troisième ligne de chimiothérapie. Deux ans après, elle a accouché d’un enfant vivant à 32semaines d’aménorrhée (SA) d’une grossesse spontanée. Dans le but de préserver la fertilité, la chirurgie utérine conservatrice est une alternative à l’hystérectomie pour certaine tumeurs trophoblastiques gestationnelles chimiorésistantes sélectionnées.
Chemotherapy is the reference treatment for gestational trophoblastic neoplasia. In case of chemoresistance, hysterectomy has to be considered even in women wishing to conceive. A 31-year-old nulliparous patient presented a recurrent invasive mole, despite two regimens of chemotherapy. She underwent a partial uterine resection of an intramyometrial choriocarcinoma followed by a third-line regimen. Two years later she gave birth by cesarean section at 32weeks of amenorrhea to a healthy child after a spontaneous pregnancy. In order to preserve patient's fertility, conservative uterine surgery is an alternative to hysterectomy for selected chemoresistant gestational trophoblastic neoplasia.
