Objective. - To determine preventable risk factors associated with recurrent hospitalization of children for acute asthma attacks.Methods. - A retrospective analysis of 288 asthmatic children between 6 and 15 years of age who were hospitalized between 31 December 1993 and 1 January 2000 was carried out.Results. - Admissions for asthma were higher in boys than in girls (male to female ratio 1.8). Of 288 children, 68 (23.6%) had multiple admissions, defined as a more than one admission per year during the follow-up period. The following factors were associated significantly with recurrent hospital admissions for asthma attacks: maternal smoking (P < 0.02); frequent upper respiratory tract infection (P < 0.001); high levels of total serum IgE (P < 0.02); high peripheral blood eosinophil count (P < 0.001); atopic dermatitis (P < 0.001); and age less than 11 years (P < 0.05). Using multivariate logistic regression analysis, among others factors only maternal smoking (r = 0.24, P < 0.001) and frequent upper respiratory tract infection (r = 0.37, P < 0.001) were significant factors for recurrent admissions.Conclusion. - Prevention of maternal smoking and frequent upper respiratory tract infections may be effective in decreasing re-admission to hospital in asthmatic children.
Objectif. - Determiner les facteurs de risques responsables hospitalisations recurrentes chez les enfants asthmatiques afin fin de les prevenir.Methodes. - Une analyse retrospective a ete realisee chez 288 enfants asthmatiques ages de 6 a 15 ans. Ces enfants ont ete hospitalises entre le 31 decembre 1993 et le 1 e r janvier 2000.Resultat. - Les hospitalisations pour crises d'asthme etaient plus frequentes chez les garcons que chez les filles (ratio garcons/filles : 1.8). Parmi les 288 enfants, 68 (23 %) ont eu des hospitalisations multiples. Une hospitalisation est multiple si l'enfant a ete admis plus d'une fois par an durant la periode de l'etude. Les facteurs suivants jouent un role significatif dans la frequence des hospitalisations pour crises d'asthme : le tabagisme maternel (p < 0,02), les infections frequentes des voies respiratoires superieures (p < 0,001), les taux eleves d'IgE seriques totales (p < 0,02), le nombre des eosinophiles dans le sang peripherique (p < 0,001), les symptomes de dermatite atopique (p < 0,001) et l'age inferieur a 11 ans (p < 0,05). Dans l'analyse de regression logistique a plusieurs variables, seuls le tabagisme maternel (r = 0,24, p < 0,001) et les infections frequentes des voies respiratoires superieures (r = 0,37, p < 0,001) ont constitue des facteurs significatifs d'hospitalisation a repetition.Conclusion. - La prevention du tabagisme maternel et des infections frequentes des voies respiratoires superieures peut s'averer une mesure efficace pour diminuer le nombre des hospitalisations chez les enfants asthmatiques.
