Le syndrome du canal carpien (SCC) chez l’hémodialysé chronique est l’ensemble des signes et symptômes liés à la compression du nerf médian dans le canal carpien, secondaire à l’amylose à bêta2microglobuline. le but de l’étude est de déterminer les facteurs de risque (FDR) du SCC chez les hémodialysés chroniques.Étude rétrospective et descriptive allant de janvier 2000 à décembre 2015, incluant 30 hémodialysés chroniques, et dont les données cliniques, biologiques et de l’électromyogramme (EMG) ont été relevées des dossiers médicaux des patients.Nous avons colligé 19 cas de SCC chez 10 malades, donnant une prévalence de 33 %. L’atteinte était du côté droit dans 5 cas, du côté gauche dans 6 cas et bilatérale dans 4 cas. Nous avons noté une nette prédominance féminine (70 %). Ces patients avaient un âge moyen de 52±13ans, avec une durée moyenne en dialyse avant la survenue du SCC de 19,5±8,5ans. La néphropathie initiale prédominante était les glomérulopathies chroniques (50 %), suivies des néphropathies indéterminées (30 %) puis la néphroangiosclérose (20 %). Les signes fonctionnels étaient essentiellement des paresthésies nocturnes et des douleurs du membre atteint pendant la séance de dialyse. Nous avons noté 10 cas (52,7 %) du SCC du côté de la fistule. La bêta2microglobuline était pratiquement élevée chez tous les malades avec une moyenne de 49±24mg/L. Tous les malades avaient un EMG confirmant le SCC. Le traitement était conservateur dans un seul cas, alors qu’il était chirurgical dans les autres cas, avec deux cas de récidive (de façon bilatérale) après traitement, ayant nécessité une réintervention avec une bonne évolution.La prévalence du SCC chez l’HDC est de 9 à 32 %, elle augmente avec l’ancienneté en dialyse (à partir de la 5e année). Les facteurs de risque sont : l’âge actuel, la durée de dialyse, le sexe féminin, et un taux élevé de la bêta2microglobuline.Le SCC reste une pathologie invalidante chez l’hémodialysé nécessitant la réalisation d’un EMG chez tout patient présentant une symptomatologie typique et annuellement chez tout malade à partir de la 5e année de dialyse, afin d’en améliorer le pronostic.
