Cliniquement, l’anorexie mentale de l’adolescent est définie de façon très stricte par les critères DSM IV. D’autres symptômes cependant peuvent être d’une aide au diagnostic pour le praticien : les pensées obsédantes autour de la nourriture, l’alexithymie, l’existence d’un trouble du goût ou de la perception. Sur le plan psychopathologique, la théorie explicative actuelle prédominante est celle de la dépendance conduisant à une compréhension clinique de l’anorexie basée sur l’omnipotence du contrôle et de la maîtrise. Mais, d’autres hypothèses découlant de la psychanalyse française contemporaine peuvent être d’un apport conséquent comme le concept d’hallucination négative. Ces nouvelles théories nous aident à concevoir d’autres modèles thérapeutiques. Actuellement, il n’y a pas d’accord sur des guidelines thérapeutiques. En France, les modèles thérapeutiques sont souvent fondés sur le contrat de poids et la séparation, modèle développé par l’équipe de Jeammet, mais reste variable d’une équipe à une autre. D’après notre expérience, nous proposons un nouveau modèle de prise en charge de l’anorexie de l’adolescent basé sur quatre grands axes : un contrat de poids sans séparation, thérapies d’inspiration analytiques de groupe et/ou individuelles, des thérapies de groupe établies sur l’écoute des signes préverbaux, des thérapies familiales individuelles ou de groupe.
Clinically, adolescent anorexia is defined by means of a rather strict manner: DMS IV criteria. However, other symptoms could be helpful for diagnosis by physicians for: obsessive thoughts about foods; alexithymia; or the existence of troubles in the sense of taste and the sense of smell. Up to today, the main psychopathogical explanation for anorexia leads to a dependence theory linked to a way of life in anorexia as determined by an omnipotent control. However, other hypothesis could be formulated to support the concept of negative hallucination developed by French contemporary psychoanalysis. These could be helpful to create a new way of treating adolescent anorexia. Actually, there are no international guidelines. In France, the way to treat anorexia is often based on a weight contract with separation developed by the “Jeammet team” and varies from one team to another. From our experiences, we propose a new way to treat anorexia based on four important axes: weight contract without separation; individual or grouped psychoanalytic psychotherapy; group therapy based on the clinical difficulty to express emotions and feelings; for families: individual and/or group therapies.
