Notre objectif est d’évaluer la spécificité analytique du test VIDAS ® Prolactine vis-à-vis de la macroprolactine circulante. L’étude comprend 86 échantillons plasmatiques dont l’hétérogénéité de la prolactine a été étudiée préalablement par chromatographie par gel-filtration : 45 échantillons à forme monomère dominante et 41 échantillons à forme big-big PRL dominante (>70 %). La concentration de PRL est déterminée en parallèle avec le test VIDAS ® sur le plasma total et le surnageant de précipitation au polyéthylène glycol (PEG). La discordance entre les valeurs obtenues par RIA qui reconnaît la macroprolactinémie de manière importante et celles obtenues avec VIDAS ® confirme la faible reconnaissance de la forme big-big PRL par la technique VIDAS ® . Le pourcentage de prolactine récupéré dans le surnageant après précipitation au PEG est de 42,7±14,6 % (m±SD) pour les plasmas à big-big PRL dominante, et de 68,3±8,1 % pour les plasmas à forme monomère dominante. Il existe un chevauchement des résultats (40 à 60 % de récupération) entre les deux groupes d’échantillons. Cependant, un pourcentage de récupération de la prolactine inférieur à 40 % est en faveur d’une forme big-big PRL prédominante alors qu’un pourcentage de récupération supérieur à 60 % est en faveur d’une forme monomère prédominante.
Our aim was to evaluate the analytical specificity of the VIDAS ® Prolactin test (ref. 30410) with regard to macroprolactin. The study was performed on 86 plasma samples whose prolactin heterogeneity had been studied by gel filtration chromatography: 45 samples with a dominant monomeric form and 41 samples with a dominant big-big PRL form. PRL concentrations were determined in parallel with the VIDAS ® test on the total plasma and on the polyethylene glycol (PEG) precipitation supernatant. Discrepancy between the values obtained with radioimmunoassay, which recognizes macroprolactinemia, and those obtained with VIDAS ® confirmed the low detection of the big-big PRL form by the VIDAS ® test. The PRL percentage recovered in the supernatant after PEG precipitation was 42.7±14.6% (m±SD) for the big-big dominant PRL plasmas, and 68.3±8.1% for the dominant monomeric form plasmas respectively. There was an overlap in the results (from 40 to 60% recovery) between the two groups of samples. Nevertheless, a recovery lower than 40% indicated a predominant big-big PRL form, whereas a recovery greater than 60% indicated a predominant monomeric form.
