A 34-year-old man was admitted after an episode of aborted sudden cardiac death. The initial investigation including electrocardiogram, chest x-ray, transthoracic echocardiogram, and biomarkers were normal. Although coronary angiography showed nonsevere stenosis, optical coherence tomography revealed severe obstruction in the artery with a layered appearance of the vessel wall; it was consistent with the presence of mural thrombus.
Un homme de 34 ans a été admis après un épisode de mort subite d’origine cardiaque avortée. L’examen initial, notamment l’électrocardiogramme, la radiographie thoracique, l’échocardiogramme transthoracique et les biomarqueurs, était normal. Bien que l’angiographie coronarienne ait montré une sténose bénigne, la tomographie en cohérence optique a révélé une obstruction grave de l’artère donnant à la paroi vasculaire un aspect multicouche qui correspondait à la présence du thrombus mural.
