L'EEG est un examen irremplacable dans le bilan d'un etat de mal epileptique convulsif. Plusieurs techniques sont possibles : enregistrement conventionnel sur papier ou enregistrement numerique a 4, 8, 10 ou 16 derivations, monitorage EEG continu avec ou sans complement video simultane, moniteur automatise de fonction cerebrale. A la phase la plus initiale de prise en charge, l'EEG pourrait apporter d'interessants elements evolutifs et pronostiques dans les etats de mal convulsifs graves. La mise en ?uvre de l'EEG ne doit cependant pas se substituer au controle immediat des crises et pourrait etre acceleree par une optimisation de la logistique de disponibilite de cet examen. A la phase secondaire de prise en charge des etats de mal convulsifs, les indications sont absolues dans les situations suivantes : difficultes dans le controle initial de l'etat de mal, avec risque important d'evolution secondaire vers un etat de mal ? subtil ? ; etat de mal resistant necessitant une sedation puissante et/ou des curarisants afin de guider la profondeur de l'anesthesie et de surveiller la reapparition eventuelle des anomalies paroxystiques ; trouble persistant et inexplique de la conscience apres traitement en apparence efficace afin de depister un etat de mal epileptique non convulsif ; doute diagnostique afin de documenter formellement un etat de mal factice.
