Le diagnostic de l'allergie respiratoire repose sur la concordance entre l'histoire clinique et les prick-tests. Le bilan sanguin n'est pas indispensable mais doit être fait avant la mise en route d'une immunothérapie spécifique. Dans l'allergie alimentaire IgE-dépendante, la positivité des prick-tests et des IgE spécifiques permet uniquement de poser le diagnostic de sensibilisation alimentaire. Le diagnostic d'allergie alimentaire est confirmé par la positivité du test de provocation oral. Dans les formes mixtes et non IgE-médiées d'allergie alimentaire, les prick-tests doivent être complétés par des atopy patch-test aux aliments consommés couramment par l'enfant. En cas de positivité, la pertinence des tests est affirmée par l'amélioration des symptômes sous régime d'éviction. La récidive des symptômes lors d'un test de provocation oral confirmera le diagnostic d'allergie alimentaire.

In mixed IgE and cell-mediated food allergy, it is necessary to do skin prick testing and atopy patch testing with the food usually ingested by the child. If these tests are positive, they are pertinent when the food eviction improves the clinical symptoms. The release of the symptoms during the oral food challenge will confirm the diagnostic of food allergy.
