Objective: To describe clinical and radiographic features of pagetic vertebral block.Subjects and Methods: We studied prospectively 190 patients with Paget's Disease of bone. Marcelli's radiographic criteriae were used: 1) Lateral or anterior fusion of vertebral bodies with bone bridging formation between two or more endplates of involved vertebrae; 2) decrease in height of vertebral bodies; 3) Partial or total loss of intervertebral disc space; 4) persistance of osteophytic outgrowth.Results: Pagetic block was found in five patients (2.6%). Four of them were male and one female, mean age was 74-76 years. One patient complainted of lumbocrural pain, while the other three were asymptomatic. All patients had polyostotic disease, 3 with thoracic involvement and 2 with lumbar spine involvement. Only one patient presented signs consistent with DISH. Follow-up showed progressive loss of intervertebral disc space with eventual trabeculation.Conclusions: Pagetic vertebral block was unfrequent and often asymptomatic. We do not believe that coexistent DISH favours developement of pagetic vertebral block.
