La sclérodermie systémique (SCS) est une affection généralisée du tissu conjonctif, des artérioles et des microvaisseaux, caractérisée par la survenue de phénomènes de fibrose et d’oblitération vasculaire. Les atteintes viscérales conditionnent le pronostic de la maladie. Les deux composantes majeures des manifestations pulmonaires de la ScS sont l’atteinte vasculaire d’une part, avec la survenue d’une hypertension pulmonaire (HTAP), et parenchymateuse d’autre part, liée à la pneumopathie interstitielle diffuse (PID).Nous avons mené une étude rétrospective sur 38 patients ayant une sclérodermie systémique et qui présentent une pneumopathie interstitielle diffuse (PID) et ayant été hospitalisés au service de médecine interne B de l’hôpital Charles-Nicolle entre 2001 et 2015. Tous nos patients répondaient aux nouveaux critères de l’EULAR 2013 sur la sclérodermie systémique.Treize patients atteints de sclérodermie systémique avaient une pneumopathie interstitielle diffuse (PID). L’âge moyen de ces patients était de 55,76 (20–68 ans) avec une prédominance féminine (sex-ratio H/F : 0,18). Une forme diffuse de la sclérodermie était retrouvée dans 7 cas (53 %). Les symptômes révélateurs de la PID étaient représentés par la toux dans 5 cas et la dyspnée dans 9 cas. L’enquête immunologique avait retrouvé des anticorps anti-topo-isomérase1 (anti-SCL 70) dans 7 cas (53 %). Le scanner thoracique a retrouvé une fibrose avancée avec un aspect en rayon de miel dans 3 cas (23 %) et des bronchectasies dans 4 cas (30 %). L’exploration fonctionnelle respiratoire a montré un syndrome restrictif sévère dans 4 cas (30 %). Le lavage broncho-alvéolaire pratiqué chez 7 patients était normal dans 4 cas. Il a montré une alvéolite macrophagique dans 2 cas et une polynucléose éosinophile dans 1 cas. Les atteintes associées étaient représentées par l’atteinte œsophagienne dans 10 cas (76 %). Une hypertension artérielle pulmonaire était retrouvée dans 2 cas. Trois patients ont été traités par corticothérapie associée au cyclophosphamide et 2 patients par cyclophosphamide seul.La prévalence des PID au cours de la SCS est difficile à préciser et varie selon les études entre 16 et 100 % des cas. Elle complique le plus souvent les formes cutanées diffuses et apparaît alors plus précocement dans les trois premières années. Les symptômes révélateurs de la PID ne sont pas spécifiques à type de toux sèche et/ou dyspnée d’effort. Le scanner thoracique montre habituellement des lésions bilatérales et symétriques, à type de verre dépoli prédominant au niveau des bases, des régions postérieures et sous-pleurales. Il s’y associe généralement des opacités réticulées, avec parfois des bronchectasies par traction. L’EFR montre un trouble ventilatoire restrictif et une altération précoce de la DLCO.Le traitement n’est pas codifié. Le traitement des pneumopathies infiltrantes diffuses de la sclérodermie systémique n’est pas encore bien codifié. Le cyclophosphamide exerce un effet bénéfique significatif mais très modéré sur la fonction respiratoire.
