In standard Extra-Corporeal Circulation (ECC), after vena cavae cannulation or implantation of an atrio-cava canula, the quality of venous return appears to be essential to obtain a good cardio-pulmonary assistance in order to minimize the ECC deleterious effects. If venous return becomes worse, it leads to check the integrity of the system. First of all air contamination inside the tubing should be eliminated. In a second order venous obstruction and tearing of vena cavae or right cardiac chambers have to be eliminated. After unsuccessful verifications, active drainage appears to be an technical opportunity to improve quality of venous return. The venous return of ventricular assist devices presents a different approach leading to the use of specific canulae in order to get an excellent longterm quality of drainage.
