L'objectif de cette etude etait d'analyser la presentation clinique initiale d'un nombre important d'enfants ayant une thyroidite chronique auto-immune et de decrire l'evolution a long terme de la maladie.Population et methodes. - Soixante enfants ages de 11,9<space>+/-<space>2,7<space>ans ont ete etudies retrospectivement.Resultats. - Au moment du diagnostic, la majorite des enfants (57<space>% des cas) etait en situation d'hypothyroidie (compensee n<space>=<space>15, franche n<space>=<space>19). L'aspect echographique de la glande thyroide etait caracteristique avec un tissu heterogene et hypoechogene. Des nodules, dont la taille variait de 3<space>a 8<space>mm de diametre, ont ete trouves chez 17<space>% des sujets. Les formes familiales de thyroidite concernaient 23<space>% des patients (n<space>=<space>14). Dix sujets avaient une maladie auto-immune associee. L'evolution a long terme a ete analysee chez les 45<space>enfants suivis pendant une duree superieure a un an (duree moyenne de suivi 4,4<space>+/-<space>3,7<space>ans). La reevaluation de la fonction thyroidienne a montre que parmi les sujets en hypothyroidie au moment du diagnostic, 28<space>% d'entre eux avaient recupere une fonction thyroidienne normale apres un delai variable de 1,5<space>a 8,4<space>ans, et que parmi les sujets en euthyroidie au moment du diagnostic, 10<space>% d'entre eux devenaient hypothyroidiens au cours de la premiere annee de suivi. Enfin, les sujets qui etaient en hypothyroidie franche au moment du diagnostic ont tous beneficie, sous traitement par L-thyroxine, d'un rattrapage statural satisfaisant avec une taille definitive normale et proche de leur taille cible.Conclusion. - Le suivi a long terme de ces patients est necessaire avec une reevaluation periodique de la fonction thyroidienne car les modifications sont frequentes et la guerison est possible. Cette reevaluation est particulierement importante chez la jeune femme devant tout souhait de grossesse pour s'assurer de l'absence d'hypothyroidie, meme partielle, qui pourrait etre potentiellement deletere pour le developpement ulterieur de l'enfant.
Objective. - The aim of the study was to describe the clinical characteristics and long-term follow-up of a group of children with chronic autoimmune thyroiditis.Patients and methods. - Sixty children (age: 11.9<space>+/-<space>2.7<space>years) were studied in a retrospective analysis.Results. - At diagnosis, 34<space>of the children were hypothyroid (compensated hypothyroidism, n<space>=<space>15; true hypothyroidism, n<space>=<space>19) and 26<space>euthyroid. Thyroid ultrasonography showed characteristic features of heterogenicity and/or hypoechogenicity in all patients. Other autoimmune diseases were found in ten subjects (17%) and 23% had a family history of thyroid disease. At the end of the follow-up, nine of 45<space>children (20% of the cases) followed up for at least one year (mean follow-up 4.4<space>+/-<space>3.7<space>years), demonstrated a change in thyroid function. Hypothyroidism was detected in two initially euthyroid patients. Among initially hypothyroid patients, seven (28%) recovered normal thyroid function, allowing L-thyroxine withdrawal for four weeks, after 1.5<space>to 8.4<space>years. No factor was found to be predictive of the course of the disease. All subjects treated with L-thyroxine for hypothyroidism reached normal final height.Conclusion. - The natural course of chronic lymphocytic thyroiditis is variable and unpredictable. Recovery from hypothyroidism does occur and can be aided by the L-thyroxine withdrawal test. The disease requires lifelong thyroid surveillance, especially in women during pregnancy, in order to avoid a potential adverse effect on the future child.
