Les atteintes pulmonaires sont frequentes en milieu professionnel, ou l'inhalation de substances potentiellement pathogenes peut etre chronique. Dans cet article, les principales pathologies pulmonaires a mecanismes immunologiques relativement bien connus, que sont l'asthme et les alveolites allergiques extrinseques, sont decrites ainsi que leur diagnostic et leur imputabilite au milieu professionnel. L'asthme professionnel (AP), caracterise par une obstruction bronchique variable au cours du temps et/ou par une hyperreactivite bronchique, est la plus frequente des maladies respiratoires professionnelles. Les substances en cause sont tres nombreuses mais les etiologies les plus frequentes sont la farine (19%) et les isocyanates (11%). Selon que les substances en cause soit de haut ou de faible poids moleculaie, les mecanismes immunologiques en cause sont lies a la production d'anticorps IgE specifiques ou sont IgE-independants.Les alveolites allergiques extrinseques (AAE) sont des pneumopathies interstitielles et alveolaires, de mecanismes immunologiques complexes, liees a l'inhalation chronique de substances organiques. Les symptomes, l'interrogatoire professionnel, l'exploration fonctionnelle respiratoire et les signes radiologiques (en periode aigue) orientent le diagnostic. Mais le lavage broncho-alveolaire est n outil diagnostique essentiel des AAE, montrant une alveolite lymphocytaire importante avec hypercellularite intense.L'AP et les AAE peuvent donner droit a reparation soit dans le cadre d'un des tableaux de maladies professionnelles de la Securite sociale, soit par decision du Comite regional de reconnaissance des maladies professionnelles.
Occupational lung disease are frequent diseases, resulting from chronic inhalation of environmental potential antigens generated in professional activities. In this article, we describe the most important lung diseases with immune mechanisms, asthma and hypersensitivity pneumonitis, their diagnosis and their imputability to professional environment.Occupational asthma, characterized by bronchial obstruction and/or hyperresponsiveness, is the most frequent respiratory occupational disease in France, with flour (19%) and isocyanates (11%) as principal etiologic agents. According to the molecular weight of the antigens involved, asthma mechanisms are IgE mediated or not.Hypersensitivity pneumonitis, less frequent than asthma, consists in a group of interstitial diseases, with complex immune mechanisms (immune complexes, lymphocytes), which develop after chronic inhalation of organic components over a long period of time. Diagnosis in acute phase involves symptomatologic clinical signs and chronology, occupational interrogation, functional respiratory exploration, and radiologic signs. Broncho-alveolar lavage showing hypercellularity and hyperlymphocytosis is characteristic of this pathology.Asthma and hypersensitivity pneumonitis may be recognized as occupational pathologies, as described in Occupational disease tables by French social security or by a Occupational disease regional committee.
