L’objectif de ce travail était de démontrer l’apport de la tomographie monophotonique couplée au scanner (TEMP/TDM ou SPECT-CT) dans la détection de ganglions sentinelles de carcinome épidermoïde du cuir chevelu.Patient de 72ans, présentant un carcinome épidermoïde du cuir chevelu, confirmé histologiquement. Une lymphoscintigraphie a été réalisée 3h avant l’intervention. La technique de l’examen consistait en 4 injections intradermiques de 0,4 mCi de nanocolloïde - Tc99m chacune, soit une activité totale de 1,6 mCi. Une acquisition dynamique est aussitôt réalisée, suivie d’acquisitions statiques cranio-cervicales, planaires (face et profil) et TEMP/TDM centrée sur la région cranio-cervicale.Un ganglion sentinelle (GS) a été détecté, en dessus des points d’injection, sur les images planaires. Le complément TEMP/TDM a permis de rattacher le ganglion détecté à un ganglion cervical du groupe IIA et à visualiser un deuxième GS hyperfixant du groupe IIB. Les deux ganglions étaient négatifs à l’examen anatomopathologique.Le GS est actuellement largement proposé dans les tumeurs cutanées. Les difficultés de prélèvement liées à la complexité du système lymphatique de la tête et du cou rendent cette technique plus aléatoire. La TEMP/TDM a permis une meilleure localisation du GS et une amélioration de la sensibilité.
