Il n’existe pas de définition d’une poussée au cours la spondyloarthrite (SpA). L’objectif de notre travail a été d’évaluer les seuils de variations d’activité de la maladie en utilisant des index composites validés.Les patients atteints d’une SpA (critères ASAS) ayant été suivis de manière prospective au cours d’au moins deux visites ont été inclus. Les patients et le médecin ont répondu à chaque visite à la question : « considérez-vous votre SpA/patient dans un état de poussée ? ». Les variations du BASDAI et de l’ASDAS entre les visites ont été évaluées et analysées en relation avec la perception d’une poussée (oui/non). Les courbes ROC ont été construites pour déterminer les seuils de variations du BASDAI et de l’ASDAS qui étaient associées à la survenue d’une poussée entre les visites.Les patients étaient issus d’une cohorte prospective de 250 patients atteints de SpA. Au total, 99 patients ayant au moins 2 visites ont été analysés. Leurs caractéristiques étaient : 67 % d’hommes ; âge moyen 45±12ans ; ancienneté de la maladie 16±10ans ; 84 % étaient HLA-B27 positifs ; 81 % avaient une SpA purement axiale ; 56 % étaient en état de PASS ; la CRP moyenne était de 8,6±13,5mg/L ; le BASDAI moyen était de 4,3±2,2 ; l’ASDAS moyen était de 2,5±1,1. Le coefficient kappa de concordance entre patient et médecin pour la notion de poussée était de 0,68. Les principaux résultats des courbes ROC sont les suivants : une variation supérieure ou égale à 2,1 unités du BASDAI (sensibilité 59 %, spécificité 83 %), une variation supérieure ou égale à 0,8 unité de l’ASDAS-VS (sensibilité 56 %, spécificité 91 %) et une variation supérieure ou égale à 1,3 unité de l’ASDAS-CRP (sensibilité 47 %, spécificité 100 %) étaient associées à une poussée.Nous proposons des seuils de variations du BASDAI, de l’ASDAS-VS et de l’ASDAS-CRP qui sont associés à une poussée (perçue par le patient et par le médecin) ou qui pourraient la définir.
