Once other causes for anterior knee pain have been excluded, our patients with therapy resistant symptoms undergo a standardised diagnostic protocol including the measurement of the intraosseous pressure of the patella and a pain provocation test. We define the combination of a positive provocation test and increased intraosseous pressure (> 25 mmHg) as a “hypertension syndrome of the patella”. These patients with a symptomatic hypertension syndrome will then undergo a minimally invasive decompressive drilling of the distal patella. In a three year follow-up of 27 patients that underwent such a decompressive drilling of the patella, the evaluation utilising a visual analog scale revealed a pain reduction from an average of 7.6 to 2.0 cm.
