La dénutrition protéinoénergétique (DPE) est fréquente chez les patients hémodialysés chroniques. Elle est associée à une augmentation significative du risque de morbidité et de mortalité. L’origine de cette malnutrition est multifactorielle et complexe, souvent en rapport avec un déséquilibre entre les apports nutritionnels insuffisants et des besoins augmentés. En Afrique du Nord, peu d’études ont été consacrées à ce sujet, d’où l’initiative de ce travail qui a pour but d’évaluer la prévalence de la DPE chez les patients hémodialysés et la détermination des facteurs prédictifs de dénutrition au sein de cette population.Il s’agit d’une étude transversale descriptive portant sur 126 patients consentants (66 femmes et 60 hommes), recrutés au sein du service de néphrologie-dialyse-transplantation rénale du CHU Ibn-Rochd de Casablanca. Le statut nutritionnel des patients est évalué à partir des données cliniques, biologiques et anthropométriques. La DPE est déterminée selon l’International Society of Renal Nutrition and Metabolism en utilisant quatre critères : l’albuminurie, la préalbuminurie, l’indice de masse corporelle (IMC) et le taux de catabolisme protéique normalisé (nPCR). L’analyse statistique réalisée par le logiciel SPSS 22.0 est basée sur le test de χ2, les corrélations de Pearson et les régressions logistiques.L’âge moyen de nos patients était de 44,81±14,00ans, la durée moyenne de dialyse était de 12,08±5,92ans. Parmi les patients, 71,5 % étaient atteints de DPE (1 à 4 facteurs), parmi eux, 13 % souffraient de malnutrition sévère (3 à 4 facteurs). Les données biologiques ont révélé une hypoprotidémie≤60g/L dans 27 % des cas, albuminémie<38g/L dans 58 % des cas, préalbuminémie<300mg/L chez 75 % des patients, cholestérolémie total≤1,5g/L dans 53 % des cas et le HDL-C était bas chez 55,7 % des patients. Le nPCR ≤1g/kg/j dans 32 % des cas. Les données de l’impédencemétrie ont révélé : un poids moyen de 58,17±14,27kg, un IMC<23kg/m2 dans 63,5 % cas. L’apport énergétique moyen quotidien de nos patients était de 1240±420kcal/j. La ration protidique moyenne quotidienne était de 0,73±0,3g/kg/j. Quatre-vingt pour cent de nos patients ont des apports protéiques insuffisants (<1g/kg/j). Les corrélations entre les différents paramètres nutritionnels ont montré, d’une part, une corrélation entre l’albuminémie et la préalbuminémie (r=0,32 ; p=0,008) et d’autre part, une association significative entre le cholestérol total et l’albumine (r=0,32 ; p=0,008). Cependant, la régression logistique a montré que la DPE est associée aux variables : « âge » (âge≥60ans ; OR=1,15, p=0,013), « inflammation » (protéine C-réactive>6mg/L ; OR=1,9, p=0,004) et « IMC » (IMC≤18kg/m2 ; OR=1,3, p=0,014).Notre étude montre que la prévalence de la dénutrition chez les hémodialysés de notre échantillon est importante. Le diagnostic précoce et la prise en charge nutritionnelle adaptée et rigoureuse d’un état de dénutrition chez les patients hémodialysés permettent de réduire la fréquence des comorbidités leurs procurant ainsi une meilleure qualité de vie.
