Objectives. To evaluate the rationale behind incidental appendectomy during radical cystectomy. Incidental appendectomy during radical cystectomy is done to avoid the morbidity due to the future development of appendicitis and an additional operation.Methods. We performed a retrospective review of 105 patients who underwent radical cystectomy during an 8-year period (1986 to 1993). Appendectomy was...
A patient who had developed a persistent fistula between the urethra and bladder neck after its surgical closure in the construction of a continent urinary pouch and Mitrofanoff nipple was successfully treated by antegrade periurethral injection of a newly approved injectable bulking agent for stress urinary incontinence (Durasphere) to occlude the bladder neck. We believe this to be the first reported...
Objectives. To use Differential Peptide Display (DPD) technology to evaluate the patterns of low-molecular-mass peptides and small proteins in the systemic and cavernous blood taken from healthy adult male volunteers during the penile stages of flaccidity and rigidity. Results from basic research implicate a role of various peptides in the control of mammalian penile erectile tissue. Nevertheless,...
Objectives. To evaluate the prostate-specific antigen (PSA) changes and the ability of PSA doubling time (PSADT) to predict disease progression in untreated patients with clinically localized prostate cancer.Methods. A total of 104 patients with localized prostate cancer were followed up expectantly with serial PSA measurements and digital rectal examination (DRE). PSADT was calculated by linear regression...
Objectives. To compare the sensitivity and negative predictive values of frozen section analysis of pelvic lymphadenectomy in patients undergoing radical retropubic prostatectomy for prostate adenocarcinoma with the predictive power of published nomograms for metastasis to lymph nodes.Methods. A retrospective review was performed on all patients who underwent bilateral pelvic lymphadenectomy and radical...
Objectives. To determine the anatomic patterns of tumor distribution in radical prostatectomy specimens from nonpalpable prostate cancer.Methods. Tumor maps directly traced from histologic slides of 62 radical prostatectomy specimens were superimposed by a computer-assisted imaging technique to create an idealized prostate gland at three levels: apex, mid-prostate, and base. To investigate specific...
Objectives. To assess the biochemical and clinical results of postprostatectomy radiotherapy (RT) for high-risk, mostly non-rgan-confined prostate cancer.Methods. After radical prostatectomy, 66 consecutive patients received either adjuvant (n = 29) or therapeutic (n = 37) postoperative RT. Therapeutic RT was given for persistently elevated postoperative prostate-specific antigen (PSA) levels (n =...
We report a case of a mixed tumor of paratesticular rhabdomyosarcoma and an adenomatoid tumor in an elderly patient. Rhabdomyosarcoma is one of the most common childhood tumors; however, it rarely occurs in the elderly. In addition, to our knowledge, paratesticular rhabdomyosarcoma and adenomatoid tumor rarely form mixed tumors. Finally, we consider the pathogenesis of diploblastic tumors.
We describe a technique to assist in the stabilization of the costochondral junction using a small strut of synthetic mesh in patients undergoing a thoracoabdominal incision.
Objectives. To evaluate the usefulness of follow-up radiographic studies after ureteroscopy by retrospective chart review.Methods. We reviewed the charts of 118 patients who underwent 134 ureteroscopic procedures from January 1998 to November 1999.Results. Follow-up was obtained at our institution for 87 patients. The follow-up period ranged from 3 to 34 weeks (mean 7, SE +/- 0.75). Of 10 patients...
Objectives. To evaluate whether the percentage of core lengths involved with prostate cancer added clinically significant information concerning the time to postoperative prostate-specific antigen (PSA) failure in the intermediate-risk patient beyond what is provided by the percentage of positive biopsies.Methods. Cox regression multivariable analysis was performed to compare the ability of the two...
Chylous ascites secondary to malignancy is usually associated with lymphoproliferative disorders such as Hodgkin's and non-Hodgkin's lymphoma. Other neoplasms include carcinoma of the pancreas, stomach, breast, ovary, carcinoid, kidney, and prostate. Chylous ascites secondary to carcinoma of the prostate is, however, extremely rare. The prognosis in these patients is usually poor and most patients...
Objectives. To analyze the minimal incision living donor (MILD) nephrectomy and compare it with the standard open donor (SOD) nephrectomy with respect to incision size, operative time, complication rate, length of hospital stay, and recipient creatinine.Methods. Forty-five consecutive patients who underwent MILD nephrectomy were compared with 13 patients who had previously undergone SOD nephrectomy...
Objectives. To determine whether easily measurable prostate biopsy features could complement Gleason score, prostate-specific antigen (PSA), or clinical stage in predicting recurrence-free survival after prostatectomy. Information relating preoperative parameters to recurrence-free survival is needed to counsel patients with newly diagnosed prostate cancer regarding expectations for postprostatectomy...
Objectives. To report our experience with intraoperative frozen section (IFS) analysis in patients who are potential candidates for nerve-sparing surgery. Potency can be maintained in select patients who undergo radical prostatectomy using a nerve-sparing approach. However, extracapsular disease extension in the area of the neurovascular bundles may compromise adequate surgical margins in some patients...
Objectives. To investigate the results of combined suprapubic cystolithotomy followed by transurethral resection (TUR) of the prostate (TURP) or TUR of the bladder neck in patients with infravesical obstruction and massive or numerous bladder stones. We also reviewed the medical literature and compared the success, morbidity, and mortality rates of the treatment modality presented here and the nonsurgical...
Polymyositis (PM) is associated with an increased risk of malignancy. We report a case in which a 45-year-old patient with PM underwent computed tomography to search for an occult malignant process. The evaluation revealed a left renal mass that was treated successfully by laparoscopic radical nephrectomy. Pathologic evaluation revealed a clear cell renal cell carcinoma. The patient remained free...
