Motivo: A identificacao precisa dos pacientes admitidos no departamento de Emergencia, com elevado risco de um sndrome coronario agudo com possvel isquemia do miocardio e um ECG nao isquemico e problematica. Os pacientes com baixo e elevado risco podem ser identificados por ecocardiografia bi-dimensional e cintigrafia de perfusao do miocardio com o tecnesio 99-m-sestamibi, mas faltam estudos comparativos nesta area. Metodo e resultados: Foram estudados 655 doentes que entraram consecutivamente no Departamento de Emergencia (DE) nos quais se considerou existir baixo ou moderado risco para isquemia do miocardio. Com base na historia de apresentacao, exame fsico e ECG os doentes fizeram ecocardiograma e cintigafia de perfusao do miocardio nas primeiras 4 horas apos entrar no DE. Os pontos alvo do estudo foram: o diagnostico de enfarte do miocardio, a realizacao da angioplastia coronaria transluminal percutanea (PTCA) e cintigrafia de perfusao sob stress positiva foram. Ambos os procedimentos imagiologicos foram realizados em 470 pacientes no DE. A concordancia entre as duas tecnicas foi elevada (concordancia de 90%) nos pacientes que tiveram Enfarte do miocardio ou submetidos a angiografia coronaria. Conclusao: A concordancia entre as duas tecnicas imagiologicas e elevada quando usada em pacientes com possvel isquemia do miocardio. Ambas identificaram os pacientes com elevado risco que necessitaram de admissao e aqueles que puderam ter alta, de forma segura, directamente a partir do DE.
