La chirurgie valvulaire précoce de l’endocardite infectieuse, c’est-à-dire effectuée pendant l’antibiothérapie initiale, est très largement utilisée. Pourtant, cette intervention n’a jamais fait la preuve de son efficacité dans un essai clinique. Cinq études d’observation récemment publiées, toutes basées sur des analyses de propension, avaient pour objectif d’évaluer les relations entre chirurgie valvulaire et mortalité dans l’endocardite infectieuse. Or, leurs résultats sont très discordants, notamment en raison des limites méthodologiques de ces études. Ainsi, à ce jour, on ne dispose pas dans la littérature d’estimation fiable du bénéfice en termes de mortalité de la chirurgie valvulaire précoce dans l’endocardite infectieuse. En outre, cet exemple nous permet de discuter de l’intérêt des analyses de propension et d’en souligner les limites.
Early valve surgery for infective endocarditis, that is surgery performed during the course of antibiotic therapy, is widely used. However, this intervention has never been proven effective in any clinical trial. Five recently published observational studies, using propensity analysis, analyzed the relationship between surgery and mortality. But they gave conflicting results, mainly because of their methodological limitations. Thus, to date, there is no reliable estimation of early valvular surgery impact on mortality. In addition, this example allows for discussing the interest of propensity analyses and to emphasizing their limitations.
