Objective: During normovolaemic haemodilution arterial O 2 -content decreases exponentially. Nevertheless, tissue oxygenation is first maintained initially by increased organ perfusion and O 2 -extraction. As soon as these compensatory mechanisms are exhausted, myocardial ischaemia and tissue hypoxia occur at an individual 'critical' haematocrit (Hct) value. This study was conducted in order to assess whether tissue hypoxia at the critical Hct is reversed by hyperoxic ventilation with 100% O 2 . Method: Eighteen anaesthetized pigs were ventilated with room air and were hemodiluted by 1:1 exchange of blood with 6% pentastarch to their individual critical Hct (onset of myocardial ischaemia; significant ECG changes). At the critical Hct, hyperoxic ventilation was initiated. In nine complete datasets, global O 2 delivery and consumption, local tissue O 2 partial pressure (tpO 2 ) (MDO-Electrode, Eschweiler, Kiel, Germany) and organ blood flow (microsphere method) in skeletal muscle were analyzed at baseline, after haemodilution to the critical Hct and after 15 min of hyperoxic ventilation. Results: At the critical Hct (7.2+/-1.2%), tpO 2 was reduced from 23+/-3 to 10+/-2 Torr with 50% of all values in the hypoxic range (<10 Torr, all P<0.05). During hyperoxic ventilation, contribution of physically dissolved O 2 to the O 2 delivery and O 2 consumption increased by 400 and 563% (P<0.05) and instantly restored tpO 2 to 18+/-2 Torr, (hypoxic values 25%, P<0.05). Conclusion: Hyperoxic ventilation reversed tissue hypoxia at the critical Hct due to preferential utilization of plasma O 2 and allowed temporary preservation of tissue oxygenation. During haemodilution, hyperoxic ventilation might offer an effective bridge until red cells are ready for transfusion.
Objectivo: Durante a hemodiluicao normovolemica o conteudo de O 2 arterial diminui exponencialmente. Contudo, a oxigenacao tecidular e primariamente mantida inicialmente pelo aumento quer da perfusao de orgaos quer da extraccao de O 2 . Quando estes mecanismos compensatorios sao exaustos, a isquemia do miocardio e a hipoxia tecidular ocorrem a um valor de hematocrito (Hct) crtico individual. Este estudo foi realizado de forma a avaliar se a hipoxia tecidular que ocorre com Hct crtico pode ser revertida pela ventilacao hiperoxica com O 2 a 100%. Metodo: Dezoito porcos anestesiados foram ventilados com ar ambiente e hemodiluidos por uma troca 1:1 de sangue com 6% pentastarch para o seu Hct crtico individual (incio de isquemia do miocardio; alteracoes ECG significantes). A um Hct crtico, foi iniciada ventilacao hiperoxica. Em nove datas ets completos, foram analizados o aporte e consumo global de O 2 , a pressao parcial de O 2 tecidular local (tpO 2 ) (MDO-Electode, Eschweiler, Kiel, Germany) e o fluxo sanguneo dos orgaos (metodo microesfera) no musculo esqueletico a um nvel de linha base, apos hemodiluicao para um Hct crtico e apos 15 min de ventilacao hiperoxica. Resultados: A um Hct crtico (7.2+/-1.2%), a tpO 2 foi reduzida de 23+/-3 para 10+/-2 Torr com 50% de todos os valores na franja hipoxica (<10 Torr, todos P<0.05). Durante a ventilacao hiperoxica, a contribuicao do O 2 fisicamente dissolvido para o aporte de O 2 e o consumo de O 2 aumentou 400 e 563% (P<0.05) restaurando instantaneamente tp O 2 para 18+/-2 Torr, (valores hipoxicos 25%, P<0.05). Conclusao: A ventilacao hiperoxica reverte a hipoxia tecidular a um Hct crtico devido a utilizacao preferencial do O 2 plasmatico e permite uma preservacao temporaria da oxigenacao tecidular. Durante a hemodiluicao, a ventilacao hiperoxica pode oferecer uma ponte eficaz ate os globulos rubros estarem prontos para transfusao.

Al agotarse estos mecanismos compensatorios, ocurre isquemia miocardica e hipoxia tisular a un valor 'individual' de hematocrito (Hct) crtico. Este estudio se condujo para evaluar, si acaso la hipoxia tisular con el hematocrito crtico es revertida por ventilacion con oxgeno 100%. Resultados: En el Hct crtico (7.2+/-1.2%). tpO 2 fue reducida de 23+/-3 a 10+/-2 Torr con 50% de todos los valores en el rango hipoxico (<10 Torr, todos P<0.05). Durante la ventilacion hiperoxica, la contribucion de O 2 disuelto fisicamente a la entrega de O 2 y al consumo de O 2 aumento por 400 y 563% (P<0.05) y restablecio instantaneamente tpO 2 a 18+/-2 Torr, (valores hipoxicos 25%, P<0.05). Conclusion: La ventilacion hiperoxica revirtio la hipoxia tisular en el Hct crtico debido a la potencial utilizacion de O 2 en el plasma y permitio preservacion temporal de la oxigenacion tisular. Durante la hemodilucion, la ventilacion hiperoxica podra ofrecer un puente efectivo hasta que los globulos rojos esten listos para transfundir.
