La prise en charge d’une ostéite est déterminante lors de la survenue d’un trouble trophique chez les sujets diabétiques. Le traitement autrefois exclusivement chirurgical est maintenant de plus en plus souvent médical. Toutefois le taux de guérison varie entre 50 et 82 % à 12 mois sans que des éléments prédictifs de récidive aient pu clairement être identifiés.L’objectif de notre étude est d’évaluer de façon prospective l’intérêt prédictif de la scintigraphie aux leucocytes marqués à l’HMPAO-99mTc en fin de traitement sur la rémission de l’ostéite.Parmi les 34 patients pris en charge dans le service pour une ostéite dans le cadre d’un mal perforant plantaire (diagnostic clinique et radiologique) entre décembre 2009 et août 2011, 20, tous bon répondeurs à l’antibiothérapie, ont bénéficié d’une scintigraphie aux leucocytes marqués à l’arrêt du traitement avec réalisation de clichés statiques à 1h et 20h suivis d’un complément SPECT TDM à 20h. Le diagnostic de récidive a été porté sur la réapparition de l’inflammation des parties molles et la progression des signes radiologiques sur le site initialement traité.13 patients ont eu une scintigraphie négative (S-), 7 une scintigraphie positive (S +). La durée moyenne de l’antibiothérapie a été de 14,4 +/−11,7 jours (S- : 15,7 +/−14,2, S +: 11+/−7,3, p=NS). Le suivi a été de 9 +/−5 mois (S- : 10,4 +/−5,3, S +: 5,2+/−3,2, p=0,004). Il y a eu 2 perdus de vue dans le groupe S +. Il y a eu 4 récidives (S- : 1/13, S +: 3/5, p=0,01) survenant en moyenne 4 mois après l’arrêt de l’antibiothérapie (2 à 7 mois) soit un taux de rémission de 78 %.L’absence de fixation scintigraphique post thérapeutique semble être un élément pronostique intéressant pour affirmer le succès du traitement médical de l’ostéite. Ces données sont à confirmer sur une plus large cohorte et un suivi prolongé.
