Successful revision anterior cruciate ligament (ACL) reconstruction requires a highly motivated patient with realisticgoals and thoughtful preoperative planning. Numerous factors may cause graft failure. Causes of failure that are attributable to the femoral tunnel are discussed. The technical aspects of general revision ACL surgery including improving visualization, hardware removal, graft removal, graft selection, and fixation techniques are presented. Special techniques with alternatives are described to help the surgeon avoid pitfalls and successfully revise nonanatomical femoral tunnels.
