Les dernières recommandations concernant le traitement du rhumatisme psoriasique (RP) datent de 2007 pour la SFR et de 2012 pour l’EULAR. Un groupe d’experts dans le domaine des spondyloarthrites s’est réuni pour émettre de nouvelles recommandations concernant la prise en charge des spondyloarthrites, y compris le RP, en se basant sur les données de la littérature et des avis d’experts.Une analyse systématique de la littérature a été conduite du 01/12/2009 au 31/03/2013 en se basant sur Medline, Embase, Cochrane, les résumés des congrès de la SFR, de l’EULAR et de l’ACR de 2010 à 2012 et les essais thérapeutiques dans http://www.clinicaltrials.gov.Des études concernant les AINS et la corticothérapie (générale ou intra-articulaire) dans le RP manquent. L’efficacité du méthotrexate versus placebo dans le RP a été évaluée dans l’essai randomisé MIPA. Les anti-TNFα (récepteur soluble ou etanercept et anticorps monoclonaux chimérique comme l’infliximab ou humanisés comme l’adalimumab et le golimumab ou pégylés comme le certolizumab) sont les biothérapies de référence dans le traitement du RP. La rémission (ou l’obtention d’une activité minimale de la maladie) est dorénavant l’objectif à atteindre par le biais du treat to target. L’intérêt d’associer le méthotrexate à un anti-TNFα reste débattu au vu de l’expérience des registres. La rotation vers un 2 e anti-TNFα en cas d’échec du 1 er est efficace mais la réponse initiale et la survie semblent moins bonnes. De nouveaux agents, comme l’aprémilast et l’ustékinumab, sont actuellement en développement.Cette revue de la littérature a permis de formuler les nouvelles recommandations de la SFR concernant le traitement du RP.
